Roles & Responsibilities: The Patient HealthNavigator (PHN) oversees a caseload of patients with substance use disordersand acts as the primary liaison between the patient and treatment programs. ThePHN is responsible for implementing a substance use screening tool to determinea treatment plan that is unique to the needs of each patient. The PHN will betrained in motivational interviewing to engage patients about substance misuseand its treatment. The motivational interviewing training will focus on basicskills for demonstrating empathy, and increasing client motivation forengagement and retention in substance abuse treatment and care, as well as otheractivities through the use of reflective listening. The PHN will work closelywith project staff to have the most updated information about coordinatedlinkage to support services and treatment programs for substance abuse. Thisposition requires a flexible and solutions-oriented individual who is sensitiveto the needs of people with substance misuse. The PHN carries out the followingtasks to screen and guide patients in their treatment plans. Role andResponsibilities: * Conducts outreach to engage patients with HIVabout substance misuse and treatment services * Maintains centralized list of resources andreferrals to substance abuse treatment services and programs updated * Serves as the primary liaison with treatmentservice providers to ensure patient linkage process is streamlined * Works with patients to increase understandingabout substance abuse, identify goals around their use and engagement insubstance abuse treatment, and develop plans to achieve those goals * Coordinates logistics for patient treatmentplans, such as arranging appointments, making appointment reminders, arrangingtransportation, making childcare arrangements * Carries out tasks that are needed for patienttreatment plans, including accompanying patients to appointments * Assists patients with questions about coverageand the application process * Conducts health education activities (e.g.,managing stress) * Follows up with patients to ensure linkage to substanceabuse treatment services * If medication-assisted treatment is recommendedfor patients with substance misuse problems, ensures that adherence isdiscussed with patients to address potential barriers Requirements: * BA/BS Degree preferred or High School Diplomawith 2 years minimum experience, 3-5 preferred * Ability to work with diverse populations * Experience in working with people with substanceuse disorders * Ability to work individually and within a largerteam * Ability to handle multiple tasks and stressfulenvironment * Ability to communicate well in English andfluency in Spanish is strongly preferred * Ability to use common office computer software(Word, Excel,) * Ability to work within an organizationalstructure and multidisciplinary team in a health system environment Strength Through Diversity The Mount Sinai Health Systembelieves that diversity is a driver for excellence.
