Position Overview:

The Healthy Start Health Educator will support the implementation and management of the Healthy Start initiative within Harris County Public Health (HCPH). This role involves providing direct family support by administering assessments, delivering health education, resource navigation and referrals, adheres to curriculum guidelines, data entry, follow up, case management, and ensuring program objectives are met to improve maternal and child health outcomes. Services may be provided in participant homes, clinics, or through telehealth modalities.

Duties and Responsibilities:

  • Implement evidence-based health education and promotion initiatives and programs to improve health outcomes among Harris County residents, in both individual and group settings, using in-person and telehealth modalities.

  • Provide direct family support by administering assessments, resource navigation, and referrals, adhering to curriculum guidelines, data entry, follow-up, and case management. Support enrolled participants with social and health benefits for new and expecting families through a systematic approach.

  • Participate in the development and creation of perinatal health and infant safety curriculum, trainings, and presentations to meet the needs of the target population. Develop and implement training programs for community health workers, volunteers, and other stakeholders to enhance health literacy in key Harris County populations.

  • Coordinate, maintain, and revise class/program schedules, ensuring efficient delivery and alignment with program objectives.

  • Plan and conduct community outreach activities to promote Healthy Start services and recruit program participants. Develop and maintain relationships with community partners around Harris County.

  • Collect and maintain accurate records and data, ensuring compliance with confidentiality and reporting requirements. Provide monthly, quarterly, and annual program reports.

  • Assist in establishing and maintaining relationships with healthcare providers, community organizations, and other stakeholders to enhance program reach and effectiveness.

  • Identify areas for improvement in health education and support the implementation of quality improvement initiatives.

Requirements

Education:

  • Bachelors degree in Public Health, Health Education, or a related field.

Experience:

  • One (1) year of experience in health education, public health, community outreach, or a related field.

Licenses:

  • Valid TX driver's license

Knowledge, Skills and Abilities:

  • Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook)

  • Excellent Communication skills (verbal & written)

Physical Demands:

  • This role requires medium physical effort; some duties require handling of average-weight objects up to 20 pounds or some standing, walking, and bending.
