Conducts health education classes using basic to moderately difficult or varied curriculum designed for the improvement of health of Kaiser members and non-members. May conduct classes of various health related topics.
Teaches and models self-management skills, concepts, resources and tools related to health, lifestyle change and behavior modification to class participants.
Assists participants in developing specific behavioral goals.
Provides individual instruction and counseling as needed.
Assesses needs of class participants and modifies class content or teaching methods to meet those needs.
Applies evidence-based education techniques in teaching methods.
Answers questions and provides class participants with appropriate resources of information.
Arranges and coordinates room set-up, availability of materials and audio/visual equipment.
Maintains records of training activities including attendance rosters, homework assignments, class evaluations and timesheets or phone TIME system.
Administers and collects class evaluation forms and class fees.
Conducts follow-up phones calls to participants including those who missed sessions to determine reason for absence, monitor progress of participants and answer related questions.
Completes required continuous training and education such as those related to workplace safety, legal and regulatory compliance, accreditation and licenser requirements and department specific requirements.
Maintains and promotes knowledge and utilization of resources related to health, lifestyle change and behavior modification offered by Kaiser Permanente and through the community.
The Kaiser Permanente Weight Loss Management program provides overweight and obese adults with a fee-for-service medically supervised, low calorie diet with intensive behavioral change group sessions.
The venture is focused on helping patients lose significant weight and maintain their weight loss through specific, customized, and long-term follow-on care.
The Health Education Instructor/Facilitator performs an essential role in the delivery of this Medical Weight Loss program.
Works with the entire program staff to help patients lose weight and maintain weight loss.
Coordinates with MDs, front-office staff and other program staff to monitor patient progress and follow-up when necessary.
Oversees the behavioral progress of a panel of patients. Facilitates multiple weekly weight management group sessions focused on behavior change.
Prepares and delivers curriculum for these sessions.
Follows-up with patients who are absent or need additional support. Identifies educational and behavior change needs of patients by assessing patient supplied data, medical records, and other health data.
Implements instructional objectives and counseling strategies based on needs assessment.
Tracks, evaluates and documents patient progress towards identified goals.
Links to patients- primary care physician as appropriate.
Provides individual short-term or long-term counseling/coaching to promote or maintain behavior change.
Initiates weekly support phone calls with patients during the weight loss and maintenance phases.
Serves as a patient advocate in the coordination and delivery of services both within the organization and the community.
Consults with medical professionals, psycho-social professionals, and other educators and managers regarding patient care issues.
Participates in marketing activities for the program, including preparing for and leading patient orientation sessions, health fairs and in-facility presentations.
Provides in-services and training to staff members, as assigned.
Provides input into the development of program curriculum, supporting materials and activities, as assigned.
May provide case management duties for a panel of weight loss patients to ensure all patients follow required medical protocols.
Bachelor's degree OR four (4) years of experience in a directly related field to field of course instruction.
High School Diploma or General Education Development (GED) required.
License, Certification, Registration
Must possess excellent communication and interpersonal skills.
Ability to function independently and exercise professional judgment.
Demonstrated skills in teaching techniques and presentation.
Demonstrated ability to lead and facilitate diverse groups of people with understanding of group dynamics.
Demonstrated knowledge of adult learning theory and group process.
Basic knowledge of health concepts, preventive health care and behavior change.
Must be able to work in a Labor/Management Partnership environment.
Bachelor-s degree or equivalent in counseling, health education, social work, behavioral psychology or other related field.
Minimum three years- experience in counseling, teaching or health education in a clinical setting. Experience in weight loss management strongly preferred.
Excellent group facilitation skills. Ability to motivate others to change behaviors.
Knowledge of principles of educational psychology, health behavior change and adult learning theory required. Nutrition knowledge strongly preferred.
Must be able to work flexible hours. Evening hours required.
Must be willing to facilitate at all Medical Weight locations (Martinez, Pleasanton, Antioch).
