Responsibilities

Department description:

It provides in-home services to seriously emotionally disturbed youth, ages 5-18, who are at risk of being placed in psychiatric hospitals or residential treatment facilities.

Job description:

Assesses patients, develops a care plan, and coordinates care for patients with chronic medical conditions, patient education, patient advocacy, and regular evaluation of patients on caseload. Also evaluates patients' eligibility for Health Home At-Risk, Health Home and other social service programs.

Alerts members of the care team when follow-up is required and ensures efficient and successful access and linkage to the full array of physical and behavioral health services. Works under close supervision.

Licensure: Medical Assistant, LPN, or Patient Care Technician preferred. Valid driver's license may be required, as determined by operational/regional needs.

Education: High School Diploma equivalent required. Associate's or Bachelor's degree in a human services or related field preferred.

Experience: Minimum of one year experience in care management, community health, social service, or medical practice preferred.

Effective oral/written/interpersonal communication skills required. Basic computer skills required. Bilingual skills may be required as determined by operational needs.
