Health Coach & Community Outreach Coordinator

The Health Coach / Community Outreach Coordinator will identify, engage, and foster relationships with community partners and volunteers to address community health problems. Assists with the planning, coordination, promotion, and implementation of programs and activities related to the Community Health Implementation Plan. Strategic planning activities that align with the mission, vision, and goals of Tanner. As a member of the health coaching team, contacts and meets with participants to provide necessary wellness coaching in which they partner with clients and patients seeking self-directed, lasting changes, aligned with their values, which promote health and well and wellness and, thereby, enhance well-being.

Education

High School Diploma or GED

Experience

Two years of related experience. Requires working knowledge of specialized practices, equipment, and procedures.

Qualifications

• Adult education instruction experience preferred.

• Bachelor's Degree preferred. Individual work experience of at least 2 years can be used in lieu of Bachelor's degree.

• Health Coaching certification, board-eligible preferred

• Demonstrated ability to work with multiple audiences across diverse populations and environments.

• Demonstrated commitment to healthy living, physical activity, and community involvement.

• Effective communicator and public speaker.

• Must possess knowledge of community resources.

• Must possess knowledge of current health and wellness issues.

• Must possess the ability to communicate with a wide range of people to create and maintain a positive image of Tanner Health System. Must be able to work with others at all levels within the organization working in collaboration with other health system departments closely to accomplish goals.

• Proficient in use of modern technology, including communications technology (email, presentation and word processing software, hand-held devices, social media, database, data entry, etc.).

• Strong networking and relationship building skills.

• Very organized with attention to detail, interpersonal skills to foster relationships, ability to follow directions and meet time sensitive deadlines.

Office Hours M-F 8a-5p, must have flexibility for evening and weekend hours based on community classes and events as needed.
