Engages plan members in health coaching support in order to close care gaps and reduce or eliminate health risk behaviors.
Assesses member readiness to change appropriately and facilitates support based upon member's stage of change through the use of Motivational Interviewing and effective care planning.
Facilitates active shift in daily habits that result in risk reduction, improved wellness, improved HEDIS rates and Stars.
Accurately assess members risk/needs and transition members across the care continuum as needed.
Provide telephonic and in-person coaching support
Assist with developing and presenting wellness related material at HA Connections.
Serve as a back-up presenter at HA Connections as needed.
Participate in on site employer based wellness related events
Establishes an effective and respectful partnership with members with an awareness of multicultural issues and resource allocation.
Documents all communication regarding member activity concisely and clearly, based on standardized department guidelines.
Works collaboratively and as a resource within the Case Management Team to better serve plan members as necessary.
The Health Coach is responsible for engaging plan members in improved self-management and lifestyle behaviors for improved wellness and health management. A focus will be members who have documented gaps in care. The Coach will facilitate engagement in the program, enhance members' readiness to change daily lifestyle habits and adherence to evidence-based care guidelines resulting in improved overall wellness and improved clinical outcomes. The Coach will skillfully navigate dialog with members utilizing Motivation Interviewing.
