GENERAL SUMMARY/ OVERVIEW STATEMENT:

The Center for Organized Research and Education (CORE) Health lifestyle medicine program at MGH Revere is an innovative primary care delivery model that offers patients a variety of group visit opportunities to better understand and control chronic diseases and conditions - such as obesity, diabetes, hypertension, stress, and insomnia through healthy lifestyle changes. Patients with a primary care doctor at MGH are eligible to participate.

CORE Health is currently expanding services and the addition of a health coach will transform the clinical care to patients by matching physician-led group visits with high-impact health coach-led individualized care plans and visits for motivation and behavior change. As the on-site CORE Health health coach at MGH Revere, the incumbent will provide a critical service for patients planning to make healthy lifestyle changes. They will meet with patients from diverse backgrounds who are participating in CORE Health. The incumbent will develop ongoing relationships with them and support them as they work toward behavior change to improve their health and wellbeing, and will use coaching methods to build patients' capacities for sustainable lifestyle change, including increased engagement, self-awareness, motivation, mindfulness, knowledge and skills, confidence, and resilience. The incumbent will play a key role in their achievements and health outcomes and the overall success of CORE Health.

This role will report directly to the CORE Health physician co-directors.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

The Health Coach will provide competent, compassionate coaching based on biopsychosocial assessment and established best practices in the discipline of coaching. Specific patient care duties will include, but are not limited to, the following:

  • Assessing patients' age, culture, and environmental situations as well as patients' emotional, social, and physical well-being to ensure appropriate interventions.

  • Participating in the development of patient-driven, individualized care plans utilizing all available resources in the community and at MGH.

  • Using a coaching framework to help clients set concrete health goals and collaborating with patients to update their goals and care plans, as needed.

  • Supporting patients in assessing both potential barriers and strengths towards achieving health goals.

  • Communicating with patients through in-person meetings, telephone calls, electronic messaging, and video calls.

  • Providing clinically appropriate referrals, such as sources for mental health care, home care and clinical trials.

  • Utilizing appropriate motivational interviewing techniques & other evidence based coaching techniques.

  • Providing patient education regarding chronic conditions, including hypertension, diabetes, depression, and sleep disorders as well as related treatments and medications.

Indirect Clinical Work:

  • Maintaining and coordinating effective and on-going communication regarding patient needs to appropriate care team members in a timely and accurate manner, within the guidelines of patient confidentiality.

  • Scheduling regular touch points and reaching out proactively if clients are not tracking towards their goals.

  • Facilitating and securing appointments across specialties and helping patients prepare for visits.

  • Developing and tracking appropriate metrics for measuring patient progress and results.

  • Working towards developing rapport and a strong working alliance with employees/ patients and care team members.

  • Acquiring and maintaining comprehensive knowledge of services provided by MGH/Partners.

  • Developing and maintaining strong relationships with MGH professionals, MGH ancillary services and affiliated hospitals to expedite coordinated care for patients and clients.

  • Meeting regularly with CORE Health physicians and the Population Health team to identify patients not at target goals.

CORE Health Support:

  • Conducting outreach activities to patients to engage them and/or re-engage them.

  • Employing strategies to best meet the service and healthcare needs of all patient referrals and responding to patients' contacts whose needs may be highly complex.

  • Facilitating special projects, as required, such as special mailings and other outreach efforts.

  • Contributing to health updates and newsletters that are put out as requested to patients in CORE Health.

SKILLS & COMPETENCIES REQUIRED:

  • Excellent customer service skills.

  • Outstanding communication skills in all formats- phone, electronic, in-person.

  • Strong clinical and analytical skills in assessing patient health needs, including medical, social and behavioral health aspects.

  • Proven ability to handle sensitive information in a discrete manner.

  • Proven ability to work independently and as part of a team.

  • Proven ability to perform well under pressure and in an ever-changing environment.

  • Ability to multitask and change priorities to best meet patients' and teams' needs.

  • Competent with Microsoft Office desktop applications; familiar with database management software

  • Self-starter; ability to organize and prioritize tasks.

  • Competency using the Internet to conduct research.

  • Proven level of professionalism to interact effectively with all levels of administration.

  • Basic knowledge of chronic diseases, especially hypertension, hypercholesterolemia, diabetes, and pre-metabolic syndrome. Knowledge in fitness and nutrition preferred, and will potentially work with a nutritionist and physical therapist to assist in care plans.

  • Poise to represent the MGH well externally via phone, written communications and in person.

  • Ability to learn and understand medical terminology and organizational departments and divisions to effectively collaborate with CSS clinical colleagues and schedulers to facilitate the appropriate medical appointments

LICENSES, CERTIFICATIONS, and/or REGISTRATIONS:

Certifications:

Required: National certification in health coaching or health education

EDUCATION: .

Minimum Required:

High school diploma, GED or equivalent

Certificate (eligible): National Board Certification in Health and Wellness Coaching (NBC-HWC)

Preferred:

Bachelor's Degree

Master's Degree health promotion, public health, exercise science, nutrition, nursing, or other clinical degree

Other: Wellcoaches Lifestyle Medicine Coach

Field of Study/Additional Specialized Training: Health education, health promotion, exercise science, nutrition, psychology or other social science

Required:

  • 3 or more years of experience as a health coach

Preferred:

  • 5 or more years of related experience in a health care environment

WORKING CONDITIONS:

Work will be performed in an office environment at 300 Ocean Avenue, Revere as well as on-site at patient doctor visits or visits with other care providers. Occasional attendance at meetings before or after the usual workday is required
