Minimum Qualifications Permanent Transfer Candidate: Current NYS employee with one year of permanent or contingent-permanent competitive service as a Health Care Surveyor 2, G20, or community Nursing Services Consultant Home Health Services 1, G18 and eligible for transfer under Section 70.1 or the Civil Service Law OR Current NYS employee with one year of permanent or contingent-permanent competitive service in a title G18 or higher and meeting the non-competitive minimum qualifications listed below.
Non-Competitive: possession of a license and current registration as a registered professional nurse in New York State; and two years of post-licensure, specialized professional administrative or professional clinical experience.
Substitution: a bachelor's degree in nursing may substitute for one year of post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of post-licensure clinical nursing experience.
Preferred Qualifications: Ability to actively participate on an interdisciplinary team, accept constructive criticism, and display constant professionalism as a representative of the Department; excellent interview, observational, and writing skills and the ability to translate collected information into legal defensible reports. Experience with the aged and Aging population, especially in the provision of direct patient care as a human services or clinical professional, and an ability to appreciate how the aging process affects outcomes; and flexibility and dedication. Clinical knowledge and experience with Long Term Care and the aging population. Good understanding and use of Microsoft Office Suite, particularly word, excel, outlook as well as sharepoint. Excellent written and oral communication skills. Federal Surveyor Minimum Qualifications Testing certification and investigatory experience.
Duties Description The incumbent will be a critical member of the recertification team and in that capacity, will conduct annual surveys and investigate complaints from residents, facility staff, advocates, and family representatives across multiple counties. These duties will include conducting interviews, reviewing medical records, care plans, and discharge plans and performing quality assurance, conducting post survey revisits both on site and off site; managing immediate jeopardy and health emergency situations as they arise. The incumbent will write and review inspection and case summary reports for accuracy, regulatory compliance, and overall quality; review and enter data into the applicable databases electronic filing systems. Additionally, the incumbent will review facility-proposed Plans of Correction and conduct Post Survey reviews to assess compliance status.
Additional Comments This position requires travel at times to areas unserved by public transit. Federal Surveyor Minimum Qualifications Test certification is required within on year of date of employment.
Posted Salary range includes the statutory hiring rate at step 4 and job rate for PS&T Grade 20 ($81,720 - $91,381), a Downstate Adjustment of $3,400, and an additional Geographic Pay Differential of $35,000.

This position may require travel throughout New York State including overnights to areas not served by public transportation.
