Health Care QM Project Mgr

  • Medicaid HEDIS Measures Training

Subject Matter Expert (SME) for all Medicaid HEDIS Measures, training Health Plan staff on HEDIS abstraction and working with an external auditor on all things HEDIS.

Fundamental Components:

This position will be responsible for being the HEDIS measure subject matter expert. They will be conducting web-ex training to all Health Plan fulltime staff and HEDIS temps that will be preforming HEDIS abstraction.

They will be responsible for preparing the training manual and materials prior to training and submitting materials to the auditor for approval. They will be responsible for all Inter rater reliability testing of the HEDIS measure, to include tacking the scores and trending the results for opportunities for improvement. This position will also be a single point of contact Resource Manager for a list of assigned Health Plans.

BACKGROUND/EXPERIENCE desired:

(*) 5 to 7 years clinical and/or managed care experience in the healthcare industry and quality management.

(*) Project management and leadership experience.

(*) HEDIS Experience

EDUCATION

(*) The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.

At James River Home Health and Hospice, Start of Care RNs will have a geographic territory and take responsibility for supporting patients as they come home and begin to receive home health services at home. The Start of Care RN does the initial comprehensive assessment of the patient, his/her family, and the home environment to make sure the home health team can meet all the requirements of the plan of care. The Start of Care RN sets the stage for a successful home care engagement and makes sure the care team is fully informed of patients' needs and preferences. Essential Job Functions/Responsibilities Patient Care * Completes an initial, comprehensive and ongoing assessments of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). * Provides professional nursing care by utilizing all elements of nursing process. * Assesses and evaluates patient's status by: * Writing and initiating plan of care * Regularly re-evaluating patient and family/caregiver needs * Participating in revising the plan of care as necessary * * Uses health assessment data to determine nursing diagnosis. * Develops a care plan that establishes goals, based on nursing diagnosis. Includes the patient and the family in the planning process. Communication * Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate. * Communicates with community health related persons to coordinate the care plan.
