Responsibilities:

  • Meet emergency demands in a calm, competent, productive, and timely manner.
  • Appropriately interacts with participants without regard to gender and cultural variables.
  • Provide ongoing observation of all client behavior immediately reporting any potentially dangerous situation to the Program Coordinator.
  • Completes clinical documentation on participants and accompanying billing documentation on a daily basis.
  • Provide case management services to participants.
  • Ensure integrity of program's clinical standards, practices, and functions.
  • Follow agency policies and procedures.
  • Maintain documentation of case management services in the participant's records.
  • act as case management resource to other staff.
  • Develop participant treatment plans and document participant progress.
  • Provide telephone contact, collateral visits and non-medically necessary transportation.
  • Transports participants to scheduled activities.
  • Arrange referrals, assessing and obtaining entitlements and meeting basic participants needs.
  • Provide peer support services under the concept and practice of recovery.
  • Provide consultation, education and advocacy services to families and other providers.
  • Perform other duties as assigned.
  • Works with clinical staff to assure follow-through on participant treatment plans.
  • Provide regular peer support groups and individual counseling.
  • Participate in the Agency's staff development training, department training and staff meetings as scheduled.
  • Participate in staffing.
  • Medication monitoring.
  • Participate in the Agency's Performance Improvement Program.
  • Provide safe, timely transportation of clients to and from facility as assigned.
  1. Pick up participants on time.
  2. Comply with safe driving practices and follow procedures for getting emergency assistance in case of vehicle breakdown.
  3. Follow behavior management procedure to control clients whose behavior is escalating, creating a threat to the safety of self or others.
  4. Initiate First Aid/CPR if needed.

Requirements

Qualifications:

  • High School Diploma or G.E.D.
  • Must be at least 21 years of age.
  • Possess valid driver's license with acceptable driving record and reliable transportation
  • First Aid and CPR training certification.
  • Physical exam.
  • Ability to lift 50 pounds.
  • Have availablilty of phone for easy access of Agency contact.
  • Able to work flexible schedule as needed: weekends, nights, days, evenings and holidays.
  • Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.
  • Must obtain a fingerprint clearance card and pass a background check.

Responsible for the delivery of case management, advocacy and clinical services to the Seriously Mentally Ill and the Mental Health participants.<br></p> <p><strong>Responsibilities</strong></p> <p> Monitor the delivery of services and progress toward goals and objectives.</p> <p> Follow agency policies and procedures.</p> <p> Responsible for utilization of proper community resources and referrals.</p> <p> Assessing client needs by completing a thorough intake process.</p> <p> Provide case management services to SMI/MH participants.</p> <p> Completion of necessary client/program documentation and billing in a timely manner.</p> <p> Develop participant treatment plans and document participant progress.</p> <p> Ensure integrity of program’s clinical standards, practices and functions.</p> <p> Provide consultation, education and advocacy services to families and other providers.</p> <p> Follow up when case management responsibilities are transferred whether temporary or permanently.</p> <p> Participates in staffing with assigned staff and acts as a case management resource to other staff.</p> <p> Provide telephone contacts, collateral visits and non-medically necessary transportation</p> <p> Arrange referrals, assessing and obtaining entitlements and meeting basic participant needs</p> <p> Participation in in-service/educational training as required and recommended by supervisor.</p> <p> Perform other duties as assigned.</p><p><strong>Requirements</strong></p><p><strong>Qualifications Required</strong></p> <p> HS Diploma or GED plus 4 years experience in behavioral health or combined behavioral health education and experience with at least 1 year case management experience. </p> <p> Must be at least 21 years of age or older.</p> <p> Possess valid driver’s license with acceptable driving record, telephone and reliable transportation.</p> <p> Prior experience in the behavioral health setting.</p> <p> Attend clinical staffing at treatment centers and psychiatric hospitals.</p> <p> Carries caseload including individual and group counseling.</p> <p> Certification in first aid, CPR. and CPI.</p> <p> Must be knowledgeable of benefit and entitlements process and community resources available.</p> <p> Willing to work nights and weekends as needed.</p> <p> Ability to effectively use CPI and behavior management techniques in crisis situations.</p> <p> Demonstrates ability to distinguish between alcohol and other drug-related symptoms and other pre-existing physical problems or pathologic behaviors.</p> <p> Demonstrates knowledge of how the acute and chronic nature of the chemical/co-dependence, and the ways medical, emotional, behavioral, social, educations, and other interventions are used to treat the participant.</p> <p> Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.</p> <p><strong>Qualifications Preferred</strong></p> <p> Bilingual – English/Spanish.</p> <p> Experience working in Integrated Healthcare, Behavioral Health or Primary Care environment.</p> <p> Advanced knowledge of cultural diversity and sensitivity.</p> <p><br></p><p><strong>Benefits</strong></p><p>Generous Benefits Package</p><p>11 paid Holidays, choice of three (3) Medical plans, Retirement Plan (401k), Life &amp; AD&amp;D Plan, Dental, Vision, Short/Long Term Disability, Tuition Reimbursement, and loan repayment through the National Health Service Corp.</p><p>Make an investment in your potential by providing integrated healthcare that addresses the whole person and promotes wellness.</p> Horizon Health And Wellness Oracle AZ
