Salary: Starting at $85,000 Actual salary may fall outside of the posted salary range as it is dependent on skills, license, and experience.

Schedule: Days Monday-Friday with Weekend Manager on Duty rotation

Location: In person

ABOUT THE ROLE

We are seeking a Health and Wellness Director to support our Personal Care and Memory Care community. A whole-person approach to health and wellness which includes eight dimensions of wellness: Emotional, Environmental, Health Services, Intellectual, Physical, Social, Spiritual and Vocational. YOU ARE EXCITED ABOUT THIS OPPORTUNITY BECAUSE YOU WILL…

The Health and Wellness Director overseeing the personal care and memory care services within our community, is responsible for a range of tasks including:

  • Assessing Residents' Health: evaluating the overall health of our residents and creating personalized care plans tailored to their unique needs.

  • Emergency Response: In urgent situations, provide immediate intervention to ensure residents' well-being.

  • Promoting Independence: facilitate residents' independence by offering nursing support that empowers them to maintain their autonomy.

  • Operational Excellence: plan, organize, develop, and direct our nursing services, always striving for the highest quality patient care.

  • Regulatory Compliance: ensuring strict adherence to federal, state, and local standards, promoting a safe and compliant environment.

Other duties:

  • Supervise and Educate Healthcare Personnel

  • Schedule Management as necessary

  • Assess Resident Appropriateness

  • Memory Care Program Integration

  • Documentation and Compliance

  • Quality Assurance Coordination

  • Level of Care Assessment

WE ARE EXCITED ABOUT YOU BECAUSE YOU…

  • Possess an active LPN or RN in the State of Pennsylvania

  • A minimum of two to three years' experience in a healthcare setting preferably a Personal Care Home or long-term care background.

  • A minimum of 1 year leadership experience overseeing a clinical team.

  • Possess the ability to communicate verbally and in writing effectively with residents, families, and staff.

  • Can generate a warm, friendly, and caring manner on first impression.

  • Can communicate effectively with residents, families, staff, and visitors.

  • Maintain the highest level of confidentiality.

  • Can pass a pre-employment drug screen and criminal background check.
