The Home Companion Team Leader assists in the day-to-day operations of Greencroft Community Based Services (CBS). Part of the Team Leaders time will be spent doing The Greencroft at Home (GAH) Team Leader assists in the day-to-day operations of GAH, a licensed Personal Care Service agency. Provides direct care, supervisory visits, team member training, client scheduling and service plan assessments. On call every other weekend and every other Holiday.

Essential Functions:

  • Provide day-to-day leadership/support to Home Care Attendants involving orientation, training, resolving client concerns and scheduling.

  • Conduct in home personal care and housekeeping assessments for GAH clients and create, evaluate, and update service plans for Greencroft at Home clients at regularly scheduled intervals, per GAH policy.

  • Work with clients, team members, and clients family to address and resolve service concerns, consulting with the Director of Community Outreach, as needed. Create, maintain, and update monthly schedules for clients and team members.

  • Complete supervised visits for each team member at regularly scheduled intervals, per GAH policy.

  • Be familiar with State regulations for Personal Service Agency and educate team members accordingly

  • Assist in planning and presenting regular in-service type training for all team members.

  • In absence of Director of Community Outreach, meet with prospective clients to explain services available through Greencroft at Home and enter information into Generations software if services are initiated.

  • In absence of Director of Community Outreach, answer phone, schedule immediate transportation needs.

  • Provide direct care to clients as needed especially in the event of an unexpected absence.

Position Requirements:

  • High School diploma or GED, preferred.

  • Must be able to read, write, and follow instructions.

  • Certified Nursing Assistant/Home Health Aides encouraged to apply.

  • One years experience of housekeeper, in a home care preferred.

  • Previous experience working with the elderly, preferred.

  • Valid Drivers License and proof of car insurance required. No moving violations within the last 3 years.

  • Should enjoy working with people, be tactful, and exercise good judgement in dealing with people.

Schedule:

Full Time, on call every other weekend and holiday
