Position Summary:

Manages and provides acute, chronic, preventive, curative and rehabilitative medical care to patients and determines appropriate regimen in specialized areas such as family practice, internal medicine and geriatrics with experience in transitions of care, chronic disease, advanced goals of care discussion and palliative management.

We also are also expanding our programs and are interested in Specialty Physicians: Family Medicine, Internal Medicine, Geriatric, OBGYN and Pediatrics

Position Requirements:

Education Required (Minimum level of education): Graduate from accredited school of Medicine

Certifications/Licenses Required: Current CA MD or DO and DEA license. CPR/BLS certification. Board certified or eligible in primary care specialty

Preferred: ACLS certification

Required (Minimum level of experience): Completion of clinical training

Preferred:

  • 1 year post completion of clinical training

  • Employment in community health center

  • Experience working with elderly population

  • Verbal and Written Skills Required to Perform the Job: Bilingual (English/Spanish) preferred

  • Medical Spanish

Technical Knowledge and Skills Preferred to Perform the Job: Skin biopsy, excision of skin lesions, suturing, casting, sigmoidoscopy, and phlebotomy.

VIEW JOBS 11/5/2019 12:00:00 AM 2020-02-03T00:00 Position Summary Under the supervision of the Center Manager and day to day supervision of the Day Center Assistant Manager, the Activity Supervisor, oversees the San Diego PACE Activities Department and promotes quality, efficiency and accessibility of Activities services to PACE participants and families. The Activities Supervisor will plan, organize and implement Activities services for San Diego PACE participants and families and oversee that all Activities department staff are doing the same. Responsibilities include but are not limited to: Coordinating, supervising and implementing all day center activities, management of the Activities department, supervision of the Activities department staff, assessment of participants and teaching to participant, caregiver or other appropriate representatives. The Activities Supervisor acts as a liaison between the interdisciplinary team, caregiver representatives, and community agencies. Essential Functions of the Job: 1. Plans a comprehensive activity program tailored to the needs, interests and limitations of frail elderly persons, to promote optimum health, independence and self-car; increase or maintain social skills and self-esteem. 2. Evaluate all PACE participants' functional status in relationship to participation in recreational activities. Establishes a realistic goal and set up a treatment plan to utilize recreation therapy techniques. Completes required progress notes and assessments of PACE patients per CMS guidelines. 3. Works in collaboration with PACE IDT and other staff members to develop care plans that meet participant's activity needs, resources and interests. 4. Provide for recreational therapy opportunities; instruct family and others on therapeutic activities as indicated. 5. Provide general instruction to the principles of recreation therapy and therapeutics activities as indicated. 6. Supervise and coordinate Program Aides, Activity Aides, Community Service providers and volunteers who are assigned to the activities program by instruction, observation and open discussion. 7. Ensures a safe, clean, professional, and welcoming environment at all times 8. Develop and posts a monthly calendar of events. 9. Assists with in-service trainings and planning of community events. 10. Develops and work within an Activity Program budget that meets the Center Managers approval; submits all requests for purchasing in advance. 11. Establishes an activity volunteer program and work with community organizations to support and enhance the Activity Program. 12. Assist participants with personal care as needed. 13. Participates in Program and Policy development for the Activity department. 14. Keeps abreast of all regulations and standards to ensure compliance with orders or directives issues by duly constituted government/regulatory agencies (PACE, CMS, DHCS, etc.) 15. Meets the administrative requirements including capturing data and preparing and submitting required reports. 16. Responsible for conducting employees performance evaluations, prepare work schedule for activity coordinator, activity coordinator assistant, volunteers, etc. 17. Be proficient in and become user of EHR and responsible for data entry of all required documentation on a daily basis. Responsible for developing specific care plan that meet participants needs, interests and skills to enhance participant's health and wellbeing. Qualifications: Licensure or Certification Required: Be a certified activity coordinator or an occupational therapist, music therapist, dance therapist, art therapist, recreational therapist, or occupational therapy assistant, or, have completed at least 36 hours of training in a course designed specifically for the activity coordinator position and receive regular consultation from an occupational therapist, occupational therapy assistant or recreation therapist. Valid CA driver license and reliable transportation. Preferred: Bachelor's degree in a related field of study. Experience Required (Minimum level of experience): Have a minimum of one year of experience in a social or recreational program, preferably in a senior care setting. Supervisory experience desired. Preferred: Bachelor's degree in a related field of study. Experience Preferred: Experience in and/or knowledge of Adult Day Health Center or PACE is desirable. Verbal and Written Skills Required to Perform the Job: Excellent oral, written communication, and interpersonal skills. Bilingual: English/Spanish required Technical Knowledge and Skills Required to Perform the Job: Must be able to work effectively in a team environment. Must be able to treat geriatric patients.
