Work collaboratively as part of an interdisciplinary team that provides in-home services to older adults and individuals with disabilities. The Social Worker will make home visits, conduct comprehensive assessments and participate in the development of person-centered care plans.

Our ideal candidate will have a strong desire to work with seniors and individuals with disabilities, excellent organizational skills and be able manage constant change with a positive attitude.


  • Bachelor's degree required.

  • Massachusetts Social Work license or 2 years experience working with older adults required.

  • The ability to establish and maintain professional relationships with consumers, family members, co-workers and community professionals.

  • The ability to work both independently and as part of a team.

  • A reliable car, current driver's license and safe driving record required.

VIEW JOBS 9/30/2022 12:00:00 AM 2022-12-29T00:00 <p>The Department of Developmental Services (DDS) is excited to offer a career growth opportunity to a motivated and talented individual for the Acquired Brain Injury (ABI) and Moving Forward Plan (MFP) Human Services Coordinator I position to function in the Metro Regional Office. This Human Service Coordinator I position will be coordinating services for individuals eligible for ABI/MFP Residential waivers pursuant to Hutchinson settlement agreement. This Human Service Coordinator I is the primary contact for individuals and families served by the Department and is expected to effectively represent the Metro Regional Office by building respectful and collaborative partnerships with individuals, families, guardian, provider agencies and other stakeholder groups involved in the work of the Region and the Department.</p><p>Duties and Responsibilities (these duties are a general summary and not all inclusive):</p><ul><li><p>Leads an Interdisciplinary Team in the development, coordination, and integration of services plans for assigned individuals.</p></li><li><p>Facilitates meetings with the interdisciplinary team to develop Individual Service Plans (ISPs).</p></li><li><p>Maintains and updates written and electronic records.</p></li><li><p>Services a advocate on behalf of the individual.</p></li><li><p>Monitors and documents the provision of services and modifies service plans as needed.</p></li><li><p>Protects the privacy of information.</p></li><li><p>Ensures that the individual's legal, civil and human rights are protected</p></li><li><p>Provides information and other services to individuals and their families.</p></li></ul><p>Please Note:</p><ul><li><p>Driving will be a necessary part of this position.</p></li><li><p>Schedule: Monday - Friday, 8:30AM to 5:00PM Off: Saturday and Sunday</p></li><li><p>Interested applicants should be flexible in schedule as there may be occasional nights and weekend meetings with individuals, guardians, families and other agencies.</p></li></ul><p>Preferred Qualifications:</p><ul><li><p>Knowledge of the principles and theories of individual and family growth and development.</p></li><li><p>Knowledge and understanding of acquired brain injury and related clinical considerations.</p></li><li><p>Knowledge of the types and manifestations of development, psychological and emotional disorders for individuals with an acquired brain injury.</p></li><li><p>Knowledge of interviewing techniques.</p></li><li><p>Ability to communicate effectively through verbal and written means.</p></li><li><p>Sensitivity to religious, ethnic and linguistic diversity of individuals with intellectual disabilities/developmental disabilities and their family members.</p></li><li><p>Ability to evaluate and maintain accurate records.</p></li><li><p>Ability to deal tactfully with others, and to establish and maintain harmonious working relationships with others.</p></li><li><p>Ability to demonstrate skill in handling confidential information.</p></li><li><p>Ability to prioritize, respond to multiple demands, and manage time efficiently.</p></li><li><p>Ability to seek guidance and support and take direction from supervisors and clinical consultants when needed.</p></li><li><p>Applicants should be comfortable with a personal computer as there will be daily entries in specific data bases, HCSIS, Meditech etc.</p></li></ul><p>About the Agency:</p><p>The Department of Developmental Services is dedicated to creating, in partnership with others, innovative and genuine opportunities for individuals with intellectual disabilities to participate fully and meaningfully in, and contribute to, their communities as valued members.</p><p>Pre-Offer Process:</p><p>Recommended candidates must successfully complete pre-employment screening which includes:</p><ul><li><p>Disabled Persons Protection Commission (DPPC) Abuse Registry Search:</p></li><li><p></p></li><li><p>Criminal background check (CORI) by regulations set forth by the Executive Office of Health and Human Services.</p></li><li><p></p></li><li><p>National Background Checks, Chapter 234 of the Acts of 2014, conduct fingerprint-based check.</p></li><li><p></p></li></ul><p>Education, licensure and certifications will be verified in accordance with the Human Resources Division's Hiring Guidelines.
