The Smilow Cancer Hospital Cancer Genetics and Prevention Program offers a multidisciplinary team approach to patient care. Under the direction of the Co-Manager Genetic Counselor, and as a member of this team, the Clinical Genetics Coordinator compliments the program by coordinating care for individuals who are at risk for hereditary cancer genetic syndromes. Excellent teamwork, verbal and written communication, collaboration and evidence-based practice are required.

Responsibilities

    1. Maintain a template of a minimum of 20 new patient openings a week less prior approved paid time off or based on position of assigned hours worked per week for those employees who are not 1.0 FTE


    1. Maintain compliance with Yale-New Haven Hospital mandatory training


    1. Meet Smilow Cancer Genetics and Prevention Program Genetic Counselor Coordinator Standards and Expectations


    1. Successful enrollment of genetics certification program approved by program medical leadership within two years of start date. Successful completion of genetics certification program within 3 years of start date required.


    1. Travel to satellite network clinic locations to extend cancer genetic services, as needed


    1. Required to demonstrate ability to learn and accurately apply genetic testing eligibility criteria and guidelines


    1. Continuously perform clinical tasks accurately and independently:

  • 7.1 7.1 Utilize clinical expertise to perform patient care responsibilities including: 7.1.1 Accurate and consistent collection of family history 7.1.2 Completion of pedigree modifications post visit 7.1.3 Timely and accurate documentation of clinical visit, including personal medical and family history 7.1.4 Order genetic testing for patient, including completion of insurance authorizations and/or insurance paperwork, as needed 7.1.5 Review results of diagnostic tests with program team to determine appropriate interpretation of results and medical management direction at weekly multidisciplinary case conference with program genetic counselor(s) and medical director(s) 7.1.6 Effectively communicate and disclose results of diagnostic tests to patients following clinical direction from genetic counselors and medical directors 7.1.7 Facilitate referrals, as needed 7.1.8 Develop written reports to be sent to patients and the patients? referring providers and other members of the patients? healthcare team based on medical information collected during the clinical genetics coordinator?s visits and diagnostic test outcomes. 7.1.9 Care coordination within the high-risk program including but not limited to: diagnostic screening, follow-up, lab testing and referrals. Will require well-coordinated cross department communication and scheduling to achieve and fulfill medical management plan and care goals in a timely manner.

    1. Other clinically relevant tasks may be assigned by program managers as needed

    1. Driven to improve the environment of the Smilow Cancer Genetics and Prevention Program with the aim of providing service excellence. The clinical genetics coordinator will be the interface and extension of the cancer genetics program for a select patient population to assure patients can access cancer genetics services in a timely manner. The clinical genetics coordinator will also need to demonstrate the following:

  • 10.1 10.1 Ability to proactively anticipate their workload and plan accordingly, switching easily from one task to another. 10.2 Identification of new ideas and methods for saving resources and accomplishing work most effectively and efficiently. 10.3 Learning from past experiences and ability to use this knowledge for future decision making. 10.4 Proactively consider and use a variety of approaches and interventions to complete tasks or achieve goals. 10.5 Proactively gets involved in projects without being asked or instructed to do so. 10.6 Follows through consistently on commitments and project closure. 10.7 Works well independently. 10.8 Exhibits strong organizational skills in relation to both autonomy and collaboration with other professionals. 10.9 Ability to communicate effectively with members of the administrative team within the Smilow Cancer Genetics and Prevention Program to independently resolve patient care issues.

Qualifications

EDUCATION

Bachelors Degree required in biology or closely related field.

EXPERIENCE

One year of direct patient care experience in a healthcare setting preferred.

LICENSURE

Successful completion of genetics certification program within 3 years of start date required.

SPECIAL SKILLS

Excellent communications, interpersonal, team, and organizational skills required. PC proficiency required. Ability to read computer screens, forms and other documents. Ability to follow written and oral instructions. Ability to write clearly and legibly. Ability to work in a fast paced and changing environment. Strong attention to detail. Must possess the ability to multi-task and problem solve on the spot. Excellent telephone etiquette.
