The Genetics Case Coordinator functions under the direction of the Clinical Coordinator, assigned provider, and the Medical Directors of the Genetics. The Genetics Case Coordinator will skillfully provide quality case management services to assigned patients , which may include patients scheduled in the FW clinic, outlying clinics, and those scheduled utilizing telemedicine. They will Work collaboratively with the Genetics team members and maintains optimum productivity while meeting the needs of the patients and their families.

The Case Coordinator will also be assisting in gathering data from patients for a research project.

QUALIFICATIONS:

  • Baccalaureate degree in genetics, biology, sociology, social work, psychology, child development, or other related field preferred.

  • Two years case management experience or comparable clinical experience desired.

  • Bilingual ability preferred. (Spanish)

This job provides basic health education, collects data and performs clerical duties as assigned by the Nurse Case Manager. The Case Management Coordinator also works with the health care team in support of case management goals, care transitions and health literacy to optimize patient care. Typical Duties: * Supports patients in accessing health related services, including but not limited to, obtaining and ensuring appropriate use of a medical home and overcoming barriers to obtaining needed medical care and social services. * Uses electronic resources and registries in collecting, reporting, gathering and documenting data for the case management process. Informs Nurse Case Manager of findings as outlined by protocol. * Assists with monitoring preventive health care screenings and informs Nurse Case Manager of outliers. * Serves as a point of contact by phone and in person for patients and others for follow-up activities. Performs face-to-face contacts in the patient's home, hospital, or medical home settings as needed. Local travel may be required to perform duties at multiple sites. * Identifies patients with limited literacy levels and assists with completing complex health forms, sharing medical history with providers, locating providers and services, understanding connections between behaviors and health, etc. * Assists patients with learning basic computer skills and in applying for assistance such as financial, food, and clothing. * Provides patients with appropriate materials to supplement basic health education and encourages patients to be active and engaged in their health and wellness. * Identifies barriers associated with undocumented patients while working with available support systems to transition the patient through the continuum of care. * Provides basic health education in one-on-one or small group settings using department approved educational tools and scripts. * Conducts transitional care visits with Nurse Case Manager to prepare the patient for a safe discharge and access to primary care. * Identifies need for advance directive assistance and reports finding to Nurse Case Manager or Social Worker. * May act as preceptor for newly hired Case Management Coordinators, assist with quality improvement activities, and participate in special case management projects. * Performs delegated case management and department operational duties as assigned. * Performs other related job duties as assigned. Qualifications: Required Education and Experience: * High School Diploma, GED or equivalent. * 2 plus years experience in a patient care or public health setting or * 2 plus years experience as a Medical Assistant.
