(i) a bachelor's or master's degree from an accredited college or university with major work in human resources or service delivery, and one year of full-time experience in the provision of services to a dependent adult population; or (ii) an associate degree from an accredited college or university with major work in human resources or service delivery, and three years of full-time experience in the provision of services to a dependent adult population. Position Summary The Case Manager works closely with the Executive Director to ensure that the community operates in accordance with residents' needs, government regulations, and Premier Senior Living policies and procedures. Essential Functions * Respects and maintains residents' rights and confidentiality and demonstrates a caring positive attitude toward all residents, staff, guests and services. * Coordinates a caseload, assuming primary responsibility for performing and coordinating the services and functions necessary to enhance the residents' autonomy and socialization skills and to provide them with an optimal living situation within the assisted living community. * Conducts interviews and evaluates prospective residents, tours families, applicants and service providers and introduces our program. * Develops, coordinates and/or implements social, emotional, and health care programs for residents in conjunction with service providers. Maintains accurate and timely progress notes. * Works with resident care staff to schedule annual appointments for physicals, eye exams, dental visits, hearing evaluations for the residents. * Provides or arranges transportation for residents to attend necessary appointments. * Maintain current knowledge of State Regulations and ensure compliance in all surveys conducted by licensing authority. * Adhere to confidentiality policy regarding information concerning residents and employees, HIPPA. * Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information. * Comply with the Residents' Rights and Community Policies and Procedures. * Must have compassion for and desire to work with the elderly. * Participates as part of the management team with the assessment of existing and new residents. * Participates as part of the team in marketing and sales activities including outreach, tours, new admissions, resident orientation and other duties as necessary. Skills and Abilities * Bachelor's degree with major course work in human services OR Associates degree with major course work in human services and three years of full-time work experience in a related field. * One year of full-time work experience in a related field. * Perform work tasks within the physical demand requirements of your position including standing, sitting, walking, lifting, carrying, pushing, pulling or otherwise move objects. * Must be computer literate, maintain documentation of resident records on chosen software programs. Certificates and Licenses * Prior completion of a state-approved training course, or willingness to complete a state approved training course within six months of employment. * Current valid documentation of successful completion of the "American Red Cross" or American Heart Association" CPR/First Aid Certification for Health Care Providers (BLS).
