  • Master's degree in Social Work (MSW) required with a specialization in clinical practice

Since 1990, New Reach has worked to further its mission to inspire independence for those affected by homelessness and poverty through a continuum of housing and support using the most innovative and progressive methods. Summary As part of a collaborative five-year project with Yale New Haven Health, New Reach is seeking two full-time Social Workers to provide person-centered case management to women (including youth) who are affected by homelessness and identified as having a co-occurring disorder to ensure follow-up of recommended treatment and stabilize housing. This role will help empower clients to gain recovery and resiliency through phased case management and will build a network of accessible and appropriate support services for individual participants that can range from family and neighborhood groups, to mental health therapy, substance abuse treatment, or holistic care such as nutritional services. Responsibilities for Social Worker: * Engage and enroll individuals who are chronically homeless who require treatment and recovery support services in preparation of entering permanent housing. * Provide case management via Critical Time Intervention to link with and retain clients in housing and other necessary services. * Ensure that those who need mainstream benefit programs (e.g., SSI/SSDI, TANF, SNAP, etc.) are referred to the SOAR worker. * Encourage an array of integrated services and supports for individuals with substance use disorders, mental disorders, or co-occurring mental and substance use disorders. * Provide access or referral to recovery support services designed to improve access to and retention in services, and to continue treatment gains. * Continuously prepare the client for transfer of care to permanent housing and recovery support caregivers. * Acts as primary contact person for not less than ten and no more than 15 clients/families at any given time. * Regular schedule is Monday through Friday 8:30am to 5:00pm. Must be able to flex schedule to work some evenings to meet the individual needs of clients. Qualifications for Social Worker: * BS in Social Work or Human Services related degree, MSW preferred * Experience in the provision of Social Services. * Must have valid driver's license and access to own reliable transportation with proof of insurance. * Bilingual/Bicultural preferred.
