The Lifeways-PACE Department is seeking a Full-Time Home Care Health Worker to join its team in San Francisco's Inner Richmond neighborhood. Reporting to the Center Supervisor, the Home Care Health Worker provides direct services to frail elders in their homes.

On Lok Lifeways pioneered the model of care known as "Program of All-inclusive Care for the Elderly" or PACE. Institute on Aging operates a PACE Center in the Richmond District of San Francisco. PACE allows seniors who are ill or disabled to live in their own home, whether home is a family residence, apartment, retirement village or hotel room.

IOA's PACE Center is a lively place where participants have access to medical care, social activities, exercise and meals. Participants may attend our PACE Center one to five days a week, based on the team's recommendations and the senior's needs and interests. IOA's staff members are sensitive to the needs and changes of each participant's health and emotional state. This comes from many decades of experience in senior care.

Specific Responsibilities:

  • As part of a health care team, provides personal care and assistance in people's place of residence, which will maintain and increase the ability of an older person to live safely and comfortably in their own homes. Tasks may include: homemaker service, assisting participants with personal care (colostomy care, bathing, peri-care, care of mouth, skin and hair) and assisting participants with eating; helping in and out of bed in the center; assistance in toileting (including use of bedpan); and assisting with ambulation, participant laundry, shopping, taking vital signs and collection of urine, sputum, etc.

  • Escorts and assist clients to, from and at medical appointments.

  • Provides behavioral/emotional supervision or support to participants, such as reality orientation or reminders to follow through on nursing/medical care and diet restrictions.

  • Other duties may include assisting in the centers or clinics; assistance with meal preparation, including special diets and set up for meals at the center or to take home.

  • Under supervision of professional staff, may be assigned to assist with special program/participant needs (i.e. recreation program, rehab/maintenance exercise program, and internal program committees).

  • Responsible for keeping areas of patient care clean and safe when participants are on site or in their homes.

  • Regularly reports on status and progress of participants homecare staff or scheduler.

  • Fulfills other administrative requirements of the job such as reporting, keeping statistical records, attending staff conferences and meetings as directed.

  • May participate in the training and orientation for HWs, volunteers, etc.

  • Actively participates in and encourages actions that promote good public relations with participants, their families and friends, visitors and the community.


Flexible hours. Full-Time, Short Shifts, Weekends*

  • Services are provided 7 days a week.

Required Qualifications:

  • At least one year's work experience working with the elderly or disabled adults.

  • Sincere interest and willingness to work with frail elders in a multilingual, multicultural environment.

  • Ability to work as part of an interdisciplinary team with initiative, imagination, resourcefulness and flexibility.

  • Knowledge of the community served.

  • Ability to communicate and speak clearly in English.

  • Agency-paid health exam & TB clearance must be completed before first assignment.

  • Background clearance prior to client assignment

  • Authorization to work in the U.S. and valid photo ID

Desired Qualifications:

  • Bilingual in English and Spanish, Chinese or Russian language

  • Certified Nursing Assistant (CNA) orHome Health Aide (HHA) certification in the State of California

  • Ability to drive and willing to transport clients. If hired, applicants accepting driving assignments must provide current driver's license, plus proof of auto insurance and a safe driving record.

Physical Requirements:

  • Ability to lift, transfer, push/pull, maneuver, and reposition 35 lbs.

  • Ability to reach, bend and walk.

  • Five finger manipulation.

The RN Case Manager works in collaboration with the Physician, Medical Social Worker and bedside RN to assure the timely movement of patients to the appropriate level of care to prevent unnecessary admissions or readmissions. Oversees the management of acute patient populations across the care continuum with the primary focus to provide coordinated, timely and integrated care. The RN Case Manager reports to either the Supervisor or Manager or Director of Care Coordination in each facility. The RN Case Manager has frequent contact with patients, families, physicians, the interdisciplinary team, nursing management, quality, ancillary services, third party payers and review agencies, claims and finance departments, Medical Directors, and contracted providers and community resources. MINIMUM POSITION REQUIREMENTS: Education: * BS in one of following: Nursing or Health related field. Master in nursing, case management or other health related field preferred. * National Certification in Case Management (i.e. ACM, ANA RN-BC Case Management) preferred. * Current California RN licensure required. Experience: * Minimum of three (3) years in acute medical/surgical/ED/ or critical care nursing area required. Masters of Nursing in Case Management in lieu of three (3) years in acute medical/surgical/ED/ or critical care nursing area may be considered for employment at director's discretion. * Previous Case Management experience preferred. * Experience utilizing electronic InteQual or other standardized criteria strongly preferred. * Previous leadership abilities preferred.
