Burlington County Special Services School District is seeking a qualified candidate to serve as a Transition Coordinator as part of our team.

The Transition Coordinator supports students with Intellectual and Developmental Disabilities (ID/DD) and their families, providing resources, guidance, and training to promote independence. This role involves helping students transition from public education to adult life, including post-secondary education, employment, or long-term support. Experienced candidates will possess the following qualifications:

• Knowledge of post-education adult services for individuals with Intellectual Disability (ID) and Developmental Disability (DD).

• School experience and background are preferred.

• Strong written, interpersonal, and communication skills.

• Criminal background clearance & District required pre-screen physical examination.

• Current residency in New Jersey, approved residency waiver or candidate agrees to obtain residency within one year of employment.

• Physical Requirements require prolonged periods of sitting at a desk and working on a computer, and must be able to lift up to 50 pounds at time.

• Valid, current NJ Driver's License to drive personal vehicles to various sites in and outside the District.

Key Responsibilities:

• Develop Individualized Transition Plans.

• Provide and coordinate resources and training opportunities for students, staff, and families.

• Serve as a resource for families to access community support and transition services.

• Facilitate the referral, application, and eligibility process for students and families to adult service providers, such as Vocational Rehabilitation Services (DVRS), Developmental Disabilities (DDD), and Support Coordinator agencies.
