VNA of Ohio is now hiring a LPN for Hospice in Eastern Cuyahoga and your qualifications and skill set may be an ideal match for this opportunity.
The VNA of Ohio is currently seeking a full time LPN Hospice for East Cuyahoga territory. The LPN will provide care to hospice patients in accordance with the care plan established by the interdisciplinary team. Work will be performed in a variety settings and travel to provide care is required.
Responsibilities
Principal Responsibilities:
Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
Provide direct nursing care as indicated by the plan of care under the supervision of RN Case Manager
Participate in the planning and coordination of patient care
Administers prescribed meds according to law as permitted within the scope of individual license
Provide patient/caregiver teaching as appropriate
Provide physical and emotional support to the patient/family in the terminal phases of life
Demonstrate respect and caring and maintains patient and staff confidentiality
Assess patient/family general physical and mental condition with the assigned RN Case Manager
Assists with provision of in-home respite and/or continuous care as patient need arises
Assist the patient with activities of daily living and encouraging appropriate self-care
Documents according to established agency guidelines and nursing practice standards
Instruct family or caregivers in caring for the patient as directed
Demonstrate teamwork throughout cooperation, adaptability, flexibility, communication, dependability, and support of the team goals
Communicates in an effective, accurate, and respectful manner
Adheres to the organization's policy in regards to absenteeism and appearance.
Other duties as assigned.
Qualifications
Job Competencies & Minimum Qualifications:
Graduate of an accredited school of nursing and licensed in good standing as an LPN in the State of Ohio
Current BLS certification
Experience in community health nursing providing direct patient care preferred
Strong computer skills
Excellent organizational and interpersonal skills Excellent verbal and written communication skills
Certification as a Certified Hospice and Palliative Care Nurse (CHPLN) desired
