As a Home Health Physical Therapist Assistant, you will:

  • Provide therapy services planned, delegated and supervised by the qualified Physical Therapist consistent with the patient's approved Plan of Treatment.

  • Observe patients during treatment, compiles data and report patient's progress to Physical Therapist.

  • Act as a liaison and maintains contact with patients, Physical Therapist, Clinical Manager(s), other members of the healthcare team and the community in assuring effective care coordination and appropriate resource referrals.

  • Recommend to Physical Therapist possible modification to existing physical therapy treatment plans and training needs to achieve maximum results.

  • Maintain and submit documents as required by the Company and/or facility.

  • Accurately and thoroughly document patient care observations, interventions and evaluation on the day services are rendered. Assist qualified Therapist with preparation of patient progress notes.

  • Participate in Performance Improvement activities as assigned. Communicate information that enable collection and root cause analysis of data to identify opportunities for improvement.

  • Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when recipient's right to access is clearly established and sharing of such information is in the best interest of the patient.

  • Maintain/enhance clinical practice skills by attending, participating in/conducting staff development/continuing education program.

Required Skills

Qualifications

  • Degree from an accredited Physical Therapist Assistant Program (approved by the APTA)

  • A minimum of six months physical therapist assistant experience preferred

  • Home Health experience a plus

  • Current and unrestricted PTA licensure

  • Current CPR certification

  • Good organizational and communication skills

ESSENTIAL FUNCTIONS * Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, and observation and team members.  * Provides individualized therapy treatment in accordance with plan of care. * May assist in the orientation of new caregivers and on-going in-service training and determine the qualifications and competence of department/services personnel who provide patient care services and who are not licensed independent practitioners. * Makes recommendations for relative content and scope of services and develop/implements policies and procedures that guide and support the provision of services. * Makes recommendations relative to the acquisition of new equipment and needs for space.  Participates with the manager in developing the annual budget. * Recommends and develops new services and programs in collaboration with staff. * Shares expertise with other health care professionals. Collaborates with other healthcare professionals to ensure smooth, continuous care of patients. Acts as a liaison with nursing, medical and other therapy staff to facilitate problem solving, coordination of services and acts as an educational resource. * Completes documentation accurately, timely and within professional standards. * Performs other duties as assigned.   QUALIFICATIONS (required unless otherwise noted) EDUCATION: * Graduate of a Physical Therapy program accredited by the Commission on Accreditation in Physical Therapy Education. * MS degree in Physical Therapy is preferred. EXPERIENCE/TRAINING:               * General knowledge of field and ability to utilize library resource appropriately * Previous experience as a Physical Therapist is preferred. * Specific experience in setting as described by the position posting LICENSE/CERTIFICATION:             * Current Physical Therapist license from the state Physical Therapy Licensing Board specific to location of practice * Current BLS (Basic Life Support) certification required at date of hire.
