As a Home Health LPN, you will:

  • Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirement, applicable healthcare standards, governmental laws and regulations, and Gentiva Health Services policies and procedures.

  • Provide skilled nursing interventions in the treatment of the patient/clients illness, rehabilitative needs and preventative care. Utilize a holistic approach in the provision of problem specific therapeutic interventions, teaching and training activities in accordance with the plan of care.

  • Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.

  • Confer with the RN Supervisor regarding needed changes in the Plan of Treatment. Accept verbal orders from physician were permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.

  • Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care.

  • Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team in a timely manner regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.

  • Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when the recipient's right to access is clearly established and the sharing of such information is dearly in the best interests of the patient.

  • Appropriately communicate to ensure adherence to professional standards in the provision of and availability of supplies, materials and equipment needed to safely and effectively implement the plan of care.

  • Prepare, submit and maintain documentation as required by the Company and/or facility. Visit/shift notes documented on day services are rendered.

Required Skills

Qualifications

  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.

  • Current nursing license in the practicing state.

  • Valid drivers license, auto insurance and reliable transportation.

  • Current CPR certification.

  • Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.

The position offers a chance to gain supervisory experience as you will be charged with helping our Personal Care Aides grow and develop their job skills. Candidates should be compassionate, empathetic, able to work well with others, and have a positive attitude. This job will be performed during normal business hours Monday - Friday 8:00 a - 4:30 p and is based out of our home office at 69 South St Auburn NY. Responsibilities: Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan under the direction of the Registered Nurse. Evaluates the significance of assessment findings and communicates any pertinent information about the patient's clinical status and ongoing needs to others in a timely fashion. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Demonstrates knowledge of medications and their correct administration based on age of the patient and patient's clinical position. Follows the five (5) medication rights and reduces the potential for medication errors. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Functions as a team member demonstrating collaboration with, and responsibility to patient, physician, interdisciplinary team. Adheres to all Agency, payroll, billing and documentation policies and procedures. Participates in Agency's other mandated activities, as requested by Clinical Supervisor or Director of Patient Care Services including in-services and UR/PI committee. Performs patient care responsibilities considering needs to the standard of care for patient's age. Addresses and supports cultural practices, as long as such practices do not harm others or interfere with the planned course of medical therapy. Promotes patient/family/caregiver education using various verbal and written communication techniques that take into account the patient's/family's cultural, ethnic and/or personal needs or preferences. Completes and submits necessary documentation in accordance with Agency policy and applicable regulations. Submits verification of completed visits in accordance with Agency policy. Participates in interdisciplinary team conferences in accordance with Agency policy. Assist with agency investigations as needed. Maintains current knowledge of community resources. Identifies and assesses appropriate external and internal resources to meet patient and/or family needs and to facilitate optimal patient outcomes while home care services are being procedures, and for transfer and discharge processes.
