Responsibilities

Responsible for providing direct patient care under the supervision of a registered nurse. Responsible for following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Follows the plan of care, providing treatments and working collaboratively with the members of the team to help meet positive patient care outcomes. Provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care. Implements current nursing practices following a comprehensive assessment and plan of care. Provides accurate and timely documentation of patient services to reflect the plan of care. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self-plan of care. Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include Hospice Aide, OT, PT, SW, ST, Dietician or Clinical Supervisor. Uses and prepares equipment and supplies effectively, adhering to aseptic techniques. Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications. Participates in personal, professional growth and development. Also participates in organization's quality assessment performance improvement teams and activities.

Qualifications

Education and ExperienceGraduate of an accredited practical nurse or vocational nursing program. Minimum of one (1) year experience as a licensed nurse.

Certifications, Licenses, and Other Special RequirementsMust be licensed as an LPN/LVN in good standing in the state(s) where he/she will be providing services.Meets all additional requirements for hire including state specific requirements. Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.

Management/Decision MakingUses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.

Knowledge and SkillsHas a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has working knowledge of the organization. Expresses verbal and written communication in a clear, positive and cooperative manner. Ability to learn the hospice regulations and requirements, governmental regulations and CHAP standards. Complies with accepted professional standards and practice. Demonstrates good verbal and written communication, and organizational skills. Understands principles of pain/symptom management. Must be able to function in a stressful work environment. Ability to market and deal tactfully with customers and the community.

This role is based out of employee's home but is required to work onsite at a client/provider 50% of the time. Position is a licensed Optum BH CM position dedicated for Providence Health Plan members Primary responsibilities to include: * Focused and Intensive case management * Provide case management and support for both direct referrals and data identified referrals. This requires clinical expertise in child, adolescents and family systems; including young adults, with focus on helping the family negotiate the complexities involved with conditions such as substance use, suicidality/homicidality, major depression, autism, ADHD, eating disorders, or any other severe mental illness as well as basic physical health and wellness. * Provide case management support to identified individuals with outreach and intervention including engagement of member and/ or family making available support throughout the entire continuum of treatment. * Explanation of authorization process * Explanation of benefit coverage information * Including coordination of benefits if multiple payers * Explanation of appeals process * Complete discharge follow-up & if needed, discharge planning / support * Ensure connection with the full range of benefits provided by their employer * Provide advocacy and support to family members, including caregiver support & appropriate referral to applicable / needed resources * Promote health, wellness and optimal psychosocial functioning for member and caregivers (identify caregiver gaps, facilitate education and respite support) * Promote, coordinate and facilitate collaboration amongst all facets of the care team, physical, mental or dental health, including treating facility, provider, or facility based Care Advocate, if involved * Consider the member's needs holistically to identify gaps in care requiring intervention * Exhibit excellent customer service and clinical skills in engaging providers in collaborative planning * Create and maintain appropriate clinical records * Participate as appropriate in clinical rounds * Conducts condition specific research to meet member needs * As needed, provide feedback regarding caseload and balance of work related to continued development and enhancement of the position as described. * Visit with members/caregivers and other providers at provider office or facility visits when appropriate Required Qualifications: * Related Master's Degree (i.e., Psychology, Education, Social Work, or Counseling) * Independent licensure in a mental health field is required (LP, LPC, LCSW, LMFT) * 2 years of experience working with individuals diagnosed with development delays, mental health diagnoses, substance use disorder diagnoses, and their families * Strong clinical and coordination focus with demonstrated passion in helping families look at and negotiate the full range of complexities and services, including: school system support, local centers of excellence, state advocacy services, local chapters and support groups, family evaluation and support for work-life * Proficient with telephonic assessment and have a comfort level in making outreach calls based upon data identified referrals in order to offer assistance and support to families with children in need * Strong interpersonal skills and demonstrated ability to engage effectively with callers, colleagues, and supervisors * Computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications * Must have designated, distraction-free work space in home for workspace * Must have access to high speed internet (DSL/Cable) Preferred Qualifications: * Adapts to change, comfortable within a fast-paced innovative environment and flexible in regard to daily priorities and responsibilities * Experience in care management or managed care setting * Experience working in or coordinating services for and between medical, behavioral, EAP, work-life * Ability to work flexibly and creatively with other professional team members * Strong customer service orientation Careers with Optum.
