Executive Summary Collier Health Services, inc., dba Healthcare Network (HCN) will have fifteen AmeriCorps members who will serve as community liaisons in Immokalee and Naples. At the end of the first program year, the AmeriCorps members will be responsible for improved access to care through delivery of community health services for 1,000 community members.

In addition, the We will serve 3,500 individuals and at the end of our program, we will have increased health knowledge and improved access to medical care for 500 individuals in Collier County. Member Duties : Support Collier County, with emphasis on the underserved populations to alleviate any potential healthcare disparities that exist Translating for patients in the community so that they may receive services. Providing information and resources to individuals in the community Create patient cases for individuals that would like to see a provider. Assist in arranging transportation or other services as may be required by patients or potential patients.
