Pathway Caring for Children is actively seeking a full-time Licensed Foster Care Case Manager to support children and teens in foster care. The caseload will be dynamic, tailored to the needs of individual clients and the capacity of foster homes. This hybrid role involves working across our North Olmsted and Canton, Ohio offices as needed.
Key Responsibilities:
Regularly conduct casework sessions with each child, documenting their progress meticulously.
Guide children towards their permanency goals, including reunification, adoption, or independent living.
Work collaboratively to stabilize mental health symptoms and maintain foster care placements.
Manage the coordination of all child services.
Ensure timely and appropriate team meetings.
Help clients develop skills to enhance functioning and manage mental health symptoms.
Complete all documentation and training promptly.
Deliver Mental Health CPST services to foster children, both individually and in groups.
Provide emotional support and technical assistance to staff and foster caregivers, including the creation and management of Behavior Management plans.
Facilitate family problem-solving meetings as needed.
Offer targeted training to foster caregivers through home visits.
Be available for crisis intervention during after-hours pager duty.
Engage in ongoing in-service training to improve job performance.
Qualifications Sought:
Preferred: State of Ohio licensure in a relevant field (LSW, LISW, LPC, LPCC, CCDC, Counselor Trainee, Psychology Assistant, etc.) as per ODJFS regulations.
A Bachelor's Degree is required.
Minimum of two years' experience with children or teens with mental health issues.
Proven skill in managing crisis situations involving children.
Familiarity with the dynamics of foster and adoptive care for children.
A track record of effective interaction with diverse individuals.
Valid driver's license with a clean driving record and the ability to travel to various locations.
Proper automotive insurance coverage and a well-maintained vehicle.
Compliance with background checks as stipulated in OAC code 5101:2-5-09 and assessor standards per ODJFS 5101:2-48-06.
Cultural sensitivity and awareness in serving the diverse populations of Pathway.
Experience in working with biological families of foster children.
