The Follow-up Nurse#in the Emergency Center plays a crucial role in developing and implementing a system for follow-up care of emergency department patients. #This system, designed to survey patient satisfaction and ensure optimal patient care and performance improvement, is a testament to the importance of your role. #As a Follow-up Nurse, you will collaborate with the Director/Manager of Emergency Services to set the standards and communicate with other ancillary departments as necessary to maintain excellence in patient care. #Your extensive clinical emergency center or ambulatory experience is highly valued and will be put to good use in this role.


DUTIES AND RESPONSIBILITIES:


Accountability and Quality Management. Take positive steps to ensure that the department and hospital objectives are carried out. Ensure that the rights of the patient and family are identified and preserved through utilization of the nursing standards of care. Carries out established standards for quality patient care, continually striving to improve quality and patient compliance. Maintains ongoing communication with staff and other resource personnel to enhance the quality of care. Under the direction of the Director/Manager of Emergency Services is responsible for the department issues regarding patient care follow-up Maintains his/her personal competency. Participates in regular department meetings and conferences for an exchange of information and ideas on patient care, the unit, and hospital issues. Plays a key role in risk management by identifying actual and potential problems and makes recommendations on how to solve them by positive interaction. Liaison between nursing and medical staff. Carries out the philosophy, goals, and standards of the department. Encourages staff to strive for continuous quality improvement individually and as a unit by providing feedback. Available resource to EC when directed by director/ manager to assist in general patient care activities. Participates in Emergency Center leadership activities: meetings, projects, and initiatives Accountability for Patient Satisfaction Follow-up Train other staff members as needed to make follow-up calls and#record data. Receives and responds to patient complaints and communicates areas of concern to the#appropriate manager. Respond to patients calling the department with questions regarding their symptoms, care, instructions, progress, or therapy. Assists in providing reports and statistics as needed. Acts as a liaison between patient and their PCP when necessary to facilitate f/u care. Makes follow-up calls to patients at the request of staff members (MDs, RNs, residents) and provides feedback to that staff member regarding the#patient#s current status. Accountability for Abnormal Lab and X-ray Follow-up. Notifies PCP or EC MD. Coordinates with the infection control department to address post-exposure action plans. Maintains reports on #required reportable: gunshot wounds, stabbings, burns, and animal bites- in cooperation with the EC Operation Manager EDUCATION AND RELATED EXPERIENCE: Required: Graduate from an Accredited Professional School of Nursing. Current Massachusetts nursing license. #3 years# staff experience. Demonstrated skills required in patient care, and interpersonal relationships. Preferred: BS Degree in Nursing or equivalent Certification in Emergency Nursing. Computer skills in Microsoft Word # Excel. Competent in using the IBEX System.
