Description

Job Summary:Interprets physician prescriptions and medication orders to ensure safety, efficacy and appropriateness and maintains compliance with all regulatory requirements.

Primary Tasks:

  • Reviews prescriptions issued by physicians, or other authorized prescribers, to assure accuracy and determine formulas and ingredients needed. Reviews and monitors drug therapy that requires dosing adjustments as authorized by the physician. Assists the Chief Pharmacist with responsibilities in accordance with the organization's missions, policy and applicable laws.

  • Compounds medications using standard formulas and processes such as weighing, measuring and mixing ingredients.

  • Monitors all posting onto the patient profile in order that accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities may be realized and better patient care received. Enters data into the computer system such as patient prescribed medications and cost. Maintains patient profiles and charge system.

  • Checks patient medication cassettes prior to delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength are being dispensed.

  • Answers questions and provides information to physicians, nurses, patients and other health care professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc. Addresses complaints and resolves problems.

  • May perform activities such as medication reviews, performance improvement projects and presentations to staff as assigned.

  • Performs other duties as assigned within scope and competency of the associate.

Requirements (Education, Experience):

  • College degree in Pharmacy required.

  • Current Pharmacy License required.

  • Demonstrates ability to respond to common inquiries or complaints from customers, regulatory agencies or members of the hospital staff.

  • Demonstrates ability to apply mathematical operations to such tasks as dosage calculations, formulation of pharmaceutical preparations and data analysis.

  • Demonstrates ability to define problems, collect data, establish facts and draw valid conclusions.

  • Demonstrates ability to interpret technical instructions and deal with several abstract variables.

Licensure: All applicants must have passed the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE). Applicants also must be licensed to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Those who will be meeting the Education and Licensure requirement within the 9-month period will be tentatively qualified and referred.
