Roles & Responsibilities:

Verifies insurance benefits for patients, determines patient liability and coordinates payment of uncovered medical expenses. Assists patients with insurance and payment issues and resolves problems as necessary.

  • Obtains and verifies patients' insurance benefit information from insurance companies or private payers regarding coverage for services

  • Obtains all necessary payer authorizations and acquires all required referrals for services to be provided

  • Negotiates modifies rates with carriers

  • Communicates and informs patients of any potential patient out-of-pocket liability, and works out payment plans as necessary. May provides resources to bridge the gap between insurance coverage and patients' liability if necessary

  • Assists patients with questions or problems regarding insurance coverage and other financial matters

  • Assists in the resolution of any patient billing issues

  • Confirms patients' coverage information previously obtained from insurance companies or private payers to ensure accuracy and that there has been no change in coverage since initial confirmation

  • Maintains and completes all insurance assessment forms and agreements and ensures they are accurate for claims processing

  • Performs other related duties

Requirements:

  • Education: Associate degree in related field; Bachelors preferred, or combination of applicable experience and education

  • Experience: 1 year (higher preferred) medical insurance experience including medical office administration, benefit verification

As a Care Management Coordinator within our ACN (Ambulatory Care Network), you will be responsible for providing intensive care management for PEDIATRIC patients with special health care needs, and their families. Your practice will involve developing individualized care plans, and collaborating with an interdisciplinary team, including physicians, to provide a comprehensive approach to care. Help minimize and prevent hospital readmissions throughout our health system for patients of special needs. Guide patients as they navigate the complex world of screenings, treatments, and recovery. Follow up with patients and their families to ensure their understanding of the plan of care, and educate on how to maintain appointments, DME, and medication. Form strong and enduring bonds with patients, families and multidisciplinary colleagues. This is a Monday - Friday day shift. Preferred Criteria Leadership experience in either inpatient care management, community care management, or outpatient/ambulatory care case management Pediatric experience Required Criteria 5 years of health care experience 3 yrs of case management in an inpatient, HMO, Insurance or other outpatient setting Creative thinking and excellent interpersonal skills Bachelors degree in Nursing (BSN) Current NYS Registered Nurse (RN) license Join a hospital where employee engagement is at an all-time high.
