Halifax Health - Hospice is seeking a Field Social Worker.

Summary

The Social Worker will be responsible for the psychosocial assessment and participate in the development of an interdisciplinary plan of care, supporting the psychosocial needs of the Partners in Care Together For Kids patient/family to ensure the best possible psychosocial outcomes desired by patient and family.

Job Qualifications

  • Valid driver's license with a good driving record; personal car in good working order

  • MSW from Council on Social Work Education (CSWE) accredited school

  • Licensed eligible preferred

  • Pediatric experience

Job Duties and Responsibilities

Responsibilities include:

  • Provide and document psychosocial assessments

  • Provide psychosocial support and services to the patient/caregiver

  • Facilitate community referrals

  • Assess bereavement needs

  • Provide crisis intervention

  • Serve as a resource to the interdisciplinary team

  • Participate in education of the community regarding Partners in Care Together for Kids and Hospice care

SKILLS, EXPERIENCE AND LICENSURE:

  • Working knowledge of systems theory and family dynamics

  • Strong clinical and assessment skills, including crisis intervention and the ability to work in a team setting

  • Excellent time-management, organizational skills, self-awareness and boundaries which are modeled for others

  • Skilled in providing care for pediatric, adolescent, adult, and geriatric patients

  • Must meet criteria for employment by Halifax Health

  • Social work or counseling background, knowledge of community resources, previous experience in hospice and pediatric setting

  • One year of social work experience in a health care setting preferred

DUTIES AND RESPONSIBILTIES:

Completes all aspects of initial and subsequent psychosocial assessments of the Partners in Care Together For Kids patient and family. Psychosocial issues are identified with patient/family and addressed in the care plans as well as during interdisciplinary team conferences. Provides counseling, support and crisis intervention to patients/families during the patient's lifetime. Provision of services are documented in the electronic medical record (EMR). Referrals made as needed. Responds promptly and appropriately to ensure patient needs are met at all times. Provides information and referrals as appropriate to patients/families in need of financial assistance or community resources. Serves as a resource and support to other interdisciplinary team members. Demonstrates a positive, informative and productive working relationship with all members of the Interdisciplinary Group (IDG), internal and external customers. Professional working relationships are maintained with referral sources, physicians' offices, and all internal and external customers. Demonstrates SW duties in accordance with current best practice standards and hospital, hospice and team standards, policies and procedures. Demonstrates a knowledge of best practice in pediatric palliative care, end of life care and support. Identifies educational needs and provides appropriate training to patient/family. Maintains all company owned equipment in good working order. Participates in orientation, supervision, and education of new employees as assigned. Attends HH-H, team, and General Staff Meetings or other In-services as required. Participates in QI activities as assigned. Demonstrates professional development in the performance of their practice: Reports to work when scheduled (Refer to Attendance policy). Maintains a professional appearance that demonstrates a positive self-image (see Professional Appearance Expectations policy). Maintains patient confidentiality. Follows legal and ethical guidelines Completes program educational requirements to maintain compliance Meets educational requirements for licensure and seeks appropriate education to enhance job performance.

WORKING CONDITIONS:

Varied environments and settings including hospice General Inpatient Units, or anywhere a patient may live or visit is requested, including homes, nursing homes, schools, daycare, etc. Home settings may include variety of animals, smoking and/or limited air conditioning. Requires mobility, automobile travel to various locations and counties in and out of personal car and moderate to large amount of driving per assignment/dept. Varied shifts per dept. Dealing with children with life threatening illnesses and their families may result in greater than average stress level.

SUPERVISION:

Works under direct supervision of the Patient Care Coordinator/Manager for the assigned area and

progress is reviewed at key points. Unlicensed staff are responsible for securing their own clinical supervision

hours toward licensure.
