Field Registered Nurse

Citadel Home Care is looking for a Field Registered Nurse to join our team in Essex County!

The Home Health Nurse will be responsible for providing comprehensive and coordinated care to patients in their homes and communities. The ideal candidate will have a strong background in home health care, an understanding of state and federal regulations, and a passion for helping patients live their best lives.

You will:

• Provide direct patient care in accordance with the physician's plan of care
• Conduct patient assessments and develop individualized care plans
• Administer medications and treatments as prescribed
• Educate patients and their families on disease management and self-care techniques
• Collaborate with other healthcare professionals to coordinate and manage patient care
• Ensure accurate and timely documentation of all patient care activities
• Monitor patient progress and adjust care plans as needed

You will have:

• Valid Registered Nurse (RN) license in the state of New Jersey
• Minimum of 1 year of clinical experience in home care or a related field
• Knowledge of Medicare and Medicaid regulations
• Excellent assessment and critical thinking skills
• Strong communication and interpersonal skills
• Ability to work independently and manage time effectively
• Reliable transportation and willingness to travel within Essex County
