Were hiring a Field Care Manager for a long-term PRN opportunity in Durham, NC The Field Care Manager is responsible to manage all aspects of in-home healthcare for their assigned clients, with a strong focus on medication management, disease process management, and general health and independence.
The Care Manager will provide clinical case management services, including: health assessments, education and client advocating.
The Care Manager will coordinate specific care plans under the direct supervision of Acuitys Corporate Clinical Services Administrator.
The Care Managers workload is self-directed meaning you set your own schedule, and are comfortable functioning in a less structured work environment.
There is an expectation of a long-term PRN commitment, with an ability to work a caseload of up to 55 clients.
The Field Care Managers Primary Responsibilities Include: Complete a health assessment at the start and end of care for each client.
Develop, coordinate and deliver a care plan.
Provide ongoing medication management.
Provide outstanding client support, and maintain face-to-face contact with each client consistently.
Submit all required paperwork within 24 hours of completing any client contact or visit.
Maintain a caseload of approximately 10-55 clients.
Maintain a flexible work schedule to accommodate after hour or weekend needs of the client.
Be willing to travel up to a 30-mile radius from your home zip code (30 miles in any direction). The Field Care Managers

Required Skills:
An experienced Registered Nurse, Licensed Practical Nurse, Social Worker, or relevant healthcare professional with in-home geriatric care experience will be considered.
A minimum of 2 years experience in home healthcare or management, specifically with the geriatric population.
Ability to work with minimal supervision.
Ability to use electronic applications and software programs, including Electronic Medical Records.
Proficiency in Microsoft Word, Outlook and Excel.
A home computer with in-home internet.
A valid drivers license, car insurance and personal vehicle.

Three Core Responsibilities of the Implementation Project Manager Role: Facilitate project management of the Implementation and Integration of customer point of care solutions, taking full responsibility of post close responsibilities to shorten Implementation to revenue time with goal of 60 days The Implementation Project Manager will serve as lead implementation resource providing effective and efficient project management of product installation and integration utilizing Implementation Best Practices. Coordinate involvement of customer, local i-STAT Solution Specialists and other resources for executing successful utilization and adoption of i-STAT products at an accelerated rate. Protect key base business in collaboration with field sales serving as the clinical lab resource Support i-STAT Solution Specialists with the development and implementation of strategies to secure base business and reduce loss and declining business Cultivate growth opportunities in targeted accounts working as the pre-close Lab technical specialist. Support i-STAT Solution Specialists with pre-close implementation-based activities to expand sales of company's portfolio of products in existing accounts and develop new business for targeted opportunities; engage in process of restructuring an institution's internal processes to better facilitate patient care utilizing APOC products; make educational presentations to audiences of physicians, nurses, medical directors, departmental and hospital administration concerning the most current trends in faster therapeutic turnaround times for clinicians who require immediate diagnostic results.
