All staff in positions which regularly require driving either company or personal vehicles while performing work on behalf of Community Action are required to provide Human Resources proof of a valid driver's license for a motor vehicle record check both at hire and annually thereafter. Human Resources will conduct a motor-vehicle record. These records will be held as confidential records in Human Resources. All accidents will be reported to Human Resources within 48 hours.

Position Summary:

In support of the Agency's mission, this position is responsible for conducting comprehensive eligibility assessments and coordinating resources for families with children prenatal to 6 that live in Washington County. This involves conducting extensive assessment interviews and is the first point of contact for a variety of programs in Washington County. Assessments and referrals take place in accordance with Help Me Grow (HMG) Prenatal to Three, Healthy Families (HF), Community Action Resource Eligibility Assessment (CARE), Find Child Care Oregon (FCCO). This position must accurately enter information into multiple client databases and provide additional referrals as needed. This position requires the ability to adapt to changing client needs and make independent decisions based on the presenting information. Duties also include establishing collaborative relationships with service providers and clients while achieving positive results with diverse populations.

Essential Functions and Responsibilities:

  • Conduct phone based, virtual or face-to-face comprehensive assessment interviews with households that live in Washington County.

  • Screening:

  • Assist families with screening process in a professional and trauma-informed manner.

  • Accurately process applications and accompanying documentation to verify eligibility, according to relevant program standards and agency policy and procedure.

  • Provide useful resource and referral recommendations (internal and external) to match the needs of families seeking services whenever appropriate such as pre-screening for Healthy Families.

  • Enroll individuals in the Oregon Health Plan.

  • Determine eligibility and process payments for a variety of programs in accordance with policies/procedures and navigate various complex funding, such as LIHEAP, LIHWA, OEAP, OREF, and others.

  • Provide care coordination for families, provide education and information on child development, provide advocacy and follow-up as needed.

  • Inform and educate parents and community members on how to choose quality child care and support enrollment access of publicly funded programs.

  • Provide child care reimbursements for individuals eligible for the Dislocated Worker program.

  • Present Help Me Grow Program to child care providers connected with CCR&R services.

  • Provide information and support to families applying for Employment Related Day Care.

  • Provide closed loop referrals and monitoring with referrals sources (social service provider, child welfare workers, child care programs, etc.) so that partners are aware of client's progress in receiving resources.

  • Independently allocate resources to households based on assessment results and knowledge of Community Action and community resources.

  • Determine eligibility and initiate referrals to a variety of programs in accordance with the policies and procedures of the CARE Assessment System, Child Care Resource & Referral, Community Connect, or Help Me Grow.

  • Maintain real-time, accurate, complete and confidential records including paper files and entry into multiple computer record systems/databases. Submit and prepare reports as required.

  • Maintain a comprehensive understanding of the eligibility requirements for each of Community Action's programs and community partner programs for which enrollment is regularly conducted.

  • Coordinate and promote access to community services, provide follow-up and advocacy as needed and conduct community outreach as needed.

  • Function independently as part of an interdisciplinary team with staff from partner agencies.

  • Must be able to work independently, be flexible, and be able to organize work and set priorities.

  • Attend staff meetings, trainings, and outreach events with community partners and Community Action programs to maintain a working knowledge of the program's current statuses.

  • As appropriate, coordinate with and provide general information pertaining to other community resources.

  • Strictly adhere to all safety policies and procedures at all times and respond/correct any at-risk safety issues immediately.

  • Strictly adhere to agency code of ethics and standards of workplace behavior.

  • Perform other duties as assigned to meet business needs including regular and reliable attendance and adherence to all company workplace behavior standards.

Marginal Functions and Responsibilities:

  • Participate in special projects and data collection to enhance program planning and delivery.

  • Keep informed about availability and status of social service programs serving Washington County Make client referrals as needed and coordinate with other team members to produce resource information.

  • Other marginal functions and responsibilities as assigned.

Required Education/Training/Experience:

  • Step 3 on the Oregon Registry or equivalency with a minimum of three (3) years of experience in performing eligibility determination, including phone and face-to-face customer/client intake and delivery of information. An equivalent combination of education and experience may be considered.

  • Bilingual in English and Vietnamese or Somali, both oral and written.

  • Knowledge of early childhood.

  • Ability to give presentations to groups.

  • Understanding of diverse cultural, geographic, economic, and social issues that impact families and ability to work effectively with diverse families.

  • Intermediate computer literacy in MS Word, Excel, Outlook, and software programs.

  • Experience completing data entry with a high degree of accuracy.

  • Experience de-escalating crisis situations and providing trauma-informed client services.

  • Experience coordinating with other professionals and social service agencies.

  • Ability to work effectively and collaboratively in a diverse work environment.

  • Ability to communicate effectively both orally and in writing.

  • Ability to adapt to changing client needs and make independent decisions based on those needs.

Preferred:

  • Knowledge of Washington County community resources.

  • Experience with computer databases, specifically OPUS and/or Service Point.

Required Licensing or Other Special Certifications:

  • Ability to obtain in 6 months: Oregon ONE Eligibility training, Utility assistance training, Healthy Families Oregon training, Find Child Care Oregon referral training.

Required Physical Demands of Essential Functions and Responsibilities:

  • Work is performed in a normal office environment.

  • Constant exposure to computer and keyboard with extensive phone use.

  • In normal work day, may stand/walk 2 hrs/day; may sit 5 hrs/day; may drive 1/2 hrs/day; may use hands for repetitive grasping, pushing and pulling, typing and fine manipulation 4.5 hrs/day; may use hands at or above shoulder level for _1_hrs/day.

  • In normal work day, may bend, squat, and/or climb frequently.

  • Work is light in nature, lifting maximum of 20 lbs with frequent lifting and/or carrying of objects weighing up to 20lbs.

  • May require walking or standing to a significant degree or sitting most of the time with a degree of pushing and pulling of arms and/or leg controls.
