GENERAL OVERVIEW OF KEY ROLES & RESPONSIBILITIES:

The Family Peer Specialist, an adult with lived experience as a family member of a child with behavioral health challenges will provide support to children and their families by engaging with families, incorporating their voice into treatment, and by providing resources as needed.

ESSENTIAL & CORE FUNCTIONS:

1.      Provide support to families around IBHS from the perspective of a peer

2.      Empower the family to understand their role as a member of the treatment team

3.      Share peer perspective with treatment team helping to improve understanding of parent point of view

4.      Provide support and coaching during meetings

5.      Work with families to develop natural supports

6.      Provide consumers and their families linkages to new services needed

7.      Ensures appropriate referrals to community resources and follow-up on these referrals.

8.      Consult with and documents contact with IBHS consumers’ families and significant persons

9.      Update contact information in Credible

10.  Attend interagency meetings, as needed

11.  Prevent crisis situation however when necessary deescalate children in crisis.

12.  Complete SDOH (matrix) with families

13.  Follow up with any needs identified on the SDOH providing support and connection with services

14.  Review Assessment for needs identified and connect consumer and families to services to address these needs

15.  Input outcome data into computer system

16.  Collect outcome data for consumers

17.  Identify and establish contact with community resources.

18.  Maintain the All IBHS excel resource spreadsheet by adding newly identified resources or changes in contact information

19.  Provide timely completion and follow up of Clinician and Directors requests for outreach and or connecting families to needed resources

20.  Escort families to appointments

21.  Complete home and community visits

22.  Represent program/Agency at community activities/fairs

23.  Meet service productivity expectations

24.  Complete all paperwork within specified time frames

25.  Maintain an understanding of agency policies and procedures

26.  Participate in and adhere to Individualized Training Plan

27.  Attend trainings as required by WES

28.  Attend and participate in supervision and staff meetings in accordance with regulatory standards.

29.  Adhere to WES’s Code of Ethics and comply with State Mental Health Code 

30.  Participate in continuous quality assurance/program development

31.  Comply with WES standards for service delivery

32.  Maintain consumer confidentiality

ADDITIONAL RESPONSIBILITIES:

              Performs other duties and special projects as assigned.

Requirements

PREREQUISITES & QUALIFICATIONS FOR THE POSITION:

1.      High school diploma or equivalent.

2.      Lived experience as a parent or primary caregiver who has raised or is raising a child with a mental, emotional, or behavioral health disorder. A “primary caregiver” must have or had the primary authority for making final binding decisions for such child or youth for at least one year.

3.      Full or part time, paid or volunteer experience within the past three years.
