REPORTS TO: BH Manager, Wraparound Services

SCHEDULE: Mon-Fri; 20 hours per week. On call/as needed; locations throughout Yolo County, to include home visits and work in various

community locations.

SALARY RANGE: $25.19 To 30.79 Hourly

About CommuniCare+OLE

Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY: Reports directly to the BH Manager, Wraparound. Provide support to youth and families receiving Wraparound services and performs duties as a program advocate. CommuniCare+OLE's mission emphasizes supportive, nonjudgmental care, preventive care, health education and community referrals to address the broad spectrum of health and psycho-social concerns of clients served.

EDUCATION, EXPERIENCE, TRAINING

  • High School Diploma or equivalent required.

  • Bilingual Spanish/English preferred

  • Must provide proof of current driver's license and auto

KNOWLEDGE OF WORK

The Family Partner uses personal and professional life experience (specifically as a parent/caregiver/youth involved with Wraparound Services, Foster Care, Child Welfare Services, and/or Juvenile Probation) to provide consultation to improve the effectiveness of youth/family-professional partnerships to meet the needs of the youth and families, and to participate in Wraparound services and community meetings to maximize youth/family voice, choice, and involvement throughout the service delivery process.

Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers.

Ability to handle escalated situations.

DUTIES AND RESPONSIBILITIES

Utilizes personal and professional life experience (specifically as a parent/caregiver/youth involved with Wraparound Services, Foster Care, Child Welfare Services, and/or Juvenile Probation) to provide peer support to youth and families served by the Wraparound program.

Provides youth and families new to the wraparound process with information about the process and procedures to assist them to successfully engage.

Learns the youth and family's story, culture, strengths, and concerns.

Participates in Child and Family Team (CFT) meetings to ensure access, voice and choice within the Wraparound process and support the youth and family's connection to the CFT, as necessary.

Serves as bridge to ensure that youth, family, and providers understand each other's perspective and information.

Provides a consistent source of encouragement, hope and non-judgmental, unconditional support to youth and families.

Serves as a consultant and mentor to youth and family throughout the Wraparound process, on an as needed basis.

Models effective coping techniques for youth and caregivers.

Participates in providing a variety of support services for youth and families (e.g.; support groups, prosocial activities, recognition events).

Produces and maintains accurate and timely documentation.

Works as a part of a multi-disciplinary team including Probation, Juvenile Detention Facility staff, CWS, Behavioral Health clinicians, Youth and Family Specialists, Wraparound Facilitators and other county agencies to coordinate care for youth and families as directed by manager.

Responsible for remaining within established ethical standards, including those of confidentiality.

Attend training/conferences as arranged.

Responsible for maintaining the standards as listed in the CommuniCare+OLE's Personnel Policies and Procedures Manual, and any other standards which may apply to this position.
