About the Job

Family Partner staff work collaboratively with MCI Coordinator and Clinicians to intervene and stabilize youth in crisis, with a focus on providing support to parents, helping parents navigate the mental health system, and offering their own personal experience to help educate the next steps for the youth in crisis.

Scheduled Hours
8 hours, part-time.

Responsibilities

  • Work with family towards meeting goal(s) established in an existing behavioral health treatment plan/care plan.

  • Support the parent/caregiver's capacity to ameliorate or resolve the youth's emotional or behavioral needs and strengthen their capacity to parent.

  • Work one-on-one and maintain regular frequent contact with the parent(s)/caregiver(s) in order to provide education and support throughout the care planning process, attends Care Plan Team (CPT) meetings, and may assist the parent(s)/ caregiver(s) in articulating the youth's strengths, needs, and goals for ICC to the care coordinator and CPT.

  • Educate parents/ caregivers about how to effectively navigate the child-serving systems for themselves and about the existence of informal/community resources available to them; and facilitates the parent's/caregiver's access to these resources.

  • Coordinate and communicate with members of the CBHI programs to serve as liaison between ESP and CBHI in order to support the Individual Care Plan.

  • Complete timely and accurate documentation to reflect services provided.

  • Person carries a caseload as assigned.

  • Communicate with Managed Care Entities to ensure authorization for services provided.

Qualifications

Minimum staff qualifications for a Family Partner includes:Experience as a caregiver of a youth with special needs, and preferably a youth with mental health needs; bachelor's degree in a human services field from an accredited university and one year of experience working with the target population; or associate's degree in a human service field from an accredited school and one year of experience working with children/adolescents/transition age youth; or high school diploma or GED and a minimum of two years of experience working with children/adolescents/transition age youth; and experience in navigating any of the child and family serving systems and teaching family members who are involved with the child and family serving systems.

About the Job The Project Manager is a grant-funded position with direct reporting responsibilities to the VP, ESP and Population Health, and works closely with the BCBS Foundation regarding the planning year of this project. The Project Manager is responsible for the implementation of policies, procedures, and protocols related to the project as well as oversight of day-to-day operations. The Project Manager ensures all required project components are defined, sets deadlines, assigns responsibilities, and monitors and summarizes progress of project. The Project Manager leads and directs the work of others, holding project participants accountable for deliverables, and manages all project deliverables to specified timeframes. This role is both a strategic manager/thinker, candidate should be familiar with a variety of the field's concepts, practices, and procedures and demonstrate the ability to execute projects tactically. Responsibilities * Develops and documents project plans, deadlines, and milestones in conjunction with the Director, Innovation and Strategic Planning, ESP Executive Director, Lowell ESP Director and BCBS Foundation. * Attends regularly scheduled project track meetings and agency meetings as required; documents minutes of meetings. * Manages resource allocation including providing direction and support, and tracks project deliverables using appropriate tools. * Continually monitors and reports on progress of the project to all stakeholders presenting reports defining project progress, problems and solutions. * Utilizes data to inform decisions about processes and protocols. * Enhances organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments. * Oral Communication - Speaks clearly and persuasively in positive or negative situations; Listens and gets clarification; Responds well to questions; Demonstrates group presentation skills; Participates in meetings. * Written Communication - Writes clearly and informatively; Edits work for spelling and grammar; Presents numerical data effectively; Able to read and interpret written information. Qualifications * Demonstrated success in a leadership or project management role preferred; Bachelor's degree required; Master's degree preferred.
