Job Summary

This is an exciting time to join a dynamic Sentara Community Care Team! We are looking for a flexible, Family Nurse Practitioner who wants to make a difference in our Woodbridge, VA community. We have a clinic office and two mobile clinics where we provide primary care services to uninsured and Medicaid patients. Some evenings and weekend hours are required. The ability to speak Spanish is extremely helpful in this role, but not required. We look forward to adding a new Nurse Practitioner to our family!

Position Highlights:

●Location: Woodbridge, VA

●Experience as a licensed provider treating family medicine, pediatrics, and adults with complex issues

●Full-time position with flexible hours and 1-2 Saturdays per month

●Shared call schedule, with RN first call

●Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients' health holistically.

●Review quality and outcome metrics monthly with the healthcare team

●Collaborate with local civic and faith-based leaders on community education programs

●Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice

MLD - Master's Level Degree: Nurse Practitioner (Required)

Advanced Cardiovascular Life Support (ACLS)
