Seeking a skillful, motivated, compassionate full-time Family Nurse Practitioner to join the San Benito Health Foundation (SBHF) Community Health Centers team. We are a thriving, growing community health center in San Benito County, providing high quality, medical, dental, vision and nutritional services to a multicultural population.

Individuals will work in a Patient Centered Medical Home Model, and work closely with a team of professionals to provide culturally sensitive, high quality, comprehensive health services in our Federally Funded Community/Migrant Health Center. Candidates must have the ability to work independently and must have valid Drivers license and be insurable.

Our FNP provides quality care to all of the patients at SBHF, participates in quality improvement, prevention activities in the community and health disparity reduction projects, and Collaborates with those responsible for case management, social services, referrals, outreach, community prevention and other SBHF managers.

Requirements

NP Degree from a recognized University
1-2 years experience working in a primary care clinic setting preferred
Current and Valid NP License
Current and Valid DEA License
Bilingual/Spanish is preferred

The LVN works directly with patients who are Incompetent to Stand Trial (IST) to provide their medications, to gather vitals, conduct chart reviews, and work closely with the psychiatric prescriber on the care, treatment and services for IST patients. LVN also responsible for facilitating the tele-psychiatry appointments for IST patients. Provides some restoration of competency services to IST patients in both group and individual formats.What you bring to the tableEducation<ul><li>Graduate from an accredited School of Nursing</li></ul>Experience<ul><li>One (1) or more years clinic experience preferred</li></ul>Licenses/Certifications<ul><li>Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of employment</li><li>Current CPR certification required</li></ul>What you will doKEY JOB RESPONSIBILITIES:<ul><li>As per the NPA, the LVN assists in the assessment and data collection of the physical, psychological and social dimensions of patients in the correctional setting.</li><li>Assists in planning an individual treatment plan by using available resources in planning care and consults with</li><li>RDON/ RNs and other staff as appropriate while applying knowledge and resources in planning care and patient teaching.</li><li>Gather patient vitals and provide/ document the information for the psychiatric provider</li><li>Conduct chart reviews and work closely with psychiatric prescriber on IST patient care needs</li><li>Facilitate the tele-psychiatry appointments on the patient side of the tablet/ camera, assisting the psychiatric prescriber as needed.</li><li>Provide restoration of competency services in both group and individual formats in accordance with the patient's treatment plan</li><li>Count controlled substances, syringes, needles and sharps at the beginning of each shift with another staff member and sign count logs as needed.</li><li>Implements clinical and technical aspects of care in accordance with established policies, procedures and protocols. Intervenes with proper safety techniques, procedures and standard precautions.</li><li>Responds to a code or health emergency within standard guidelines.</li><li>Implements medical plan through administering medications in accordance with Health Care Practitioner's orders and protocols: Administers medications according to proper techniques and procedures including IV therapy, pursuant to competency and all other approved routes of administration.
