Duties:

Provides assistance and support to physicians and other medical providers in assessing, planning, and providing patient care. Requires a current RN license and completion of a certified Practitioner program.

Provide assistance and support to patients enrolled in Chronic Care Program. Conduct home visits with patients to consist of an initial comprehensive evaluation, followed by subsequent visits targeting risk factors for diseases exacerbations and hospitalizations. Part of the initial and subsequently annual visit, ADL deficits, fall risk, depression, cognitive impairment and nutritional health screening must be completed. A complete medication reconciliation is to be done at each visit. Work closely with collaborative team to ensure quality care is delivered. Coordinate care with patient's other providers and case management team to ensure all are working towards the patients goals. Thorough and appropriate clinical documentation of acute and chronic healthcare issues, including symptoms and goals through patient encounter in the electronic medical record (EMR) system completed in a timely manner. Ensure consent forms are signed and dated. Educate patients and/or patient's family on chronic medical conditions, preventative care and medication adherence compliance. Participate in all clinical meetings. Collaborate with the supervising physician and other team members to ensure outstanding care is delivered. Facilitate advance care planning.

Skills:

SKILLS

Prior clinical experience in home based care and/or geriatrics is preferred. Experience with an EMR system is required. Microsoft Office proficiency is also required. Strong interest in primary care and preventive medicine. Must have a valid state driver's license. Highly self-motivated and self-directed professional with strong organizational skills and comfortable working independently on a daily basis. Excellent verbal and communication skills. Able to identify, diagnose, treat and document acute and chronic diseases in adults. Able to recognize conditions, characteristics, and behaviors that place patients at risk. Technologically savvy, as provider will be working on cell phone and or laptop.

Work 8am-5pm, occasional weekends, able to take call

Education:

Requires a current RN license and completion of a certified Practitioner program.

EDUCATION

Requires completion of an Advanced Nurse Practitioner program (Adult, Family or Geriatric). Must be board certified by the ANCC or AANP. Must have a current RN and APRN licensure in state of practice. Must have BLS certification; ACLS preferred. Possession of DEA registration or ability to obtain is required. Chronic Care Experience.
