YOUR ROLE & IMPACT
As part of the dynamic primary care team at Charles River Community Health (CRCH), a healthcare practice providing comprehensive and compassionate care to underserved communities, the Family Nurse Practitioner will be working with a dedicated, committed, diverse group of nurse practitioners, MDs, and physician assistants who collaborate with an energetic group of nurses, medical assistants, and a multidisciplinary team to provide quality care and patient-centered care coordination. The Family Nurse Practitioner will work with patients and staff who are racially, ethnically, and culturally diverse, so being culturally competent is required.
YOUR RESPONSIBILITIES
Perform holistic, evidence-based medical care services for health center patients.
Evaluate, treat, and care for patients (including walk-ins), write prescriptions, provide preventive health care advice and case management, assess psychological impact of illness, and treat or appropriately refer patients for evaluation and treatment.
Participate in student and/or other clinical discipline training activities at the health center.
Participate in the formation and evaluation of applicable clinical protocols.
Carry a patient panel of a minimum of 1000 patients.
Maintain continuity of care when patients utilize other health care facilities, i.e. referrals, laboratory, emergency room services, outpatient departments, specialty clinics, and inpatient services.
Refer patients for specialty services, as appropriate, as well as provide documentation according to health centers procedures and follow up on results.
Participate in staff meetings, including quality assurance and medical audit activities in keeping with health centers procedures.
Participate in 24-hour on-call medical coverage for one week at a time/about 1-2 times per quarter, including working 1-2 Saturday mornings per quarter.
Have a standing patient care session one night per week.
Work collaboratively with other staff in meeting ACO quality goals, DPH accreditation, and other regulatory and/or funding requirements.
Perform other related duties, as required.
YOUR QUALIFICATIONS, COMPETENCIES, TRAITS
Must be a graduate of Masters of Science in Nurse Practitioner program, or have completed a Certified Nurse Practitioner program.
2 years of nurse practitioner experience is required.
Must have Massachusetts certification in Expanded Role from Board of Registration in Nursing.
Must have Massachusetts Nursing License.
Must have Nurse Practitioner Certification.
Prior ambulatory experience preferred.
Must be able to work out of either our Brighton or Waltham locations as needed.
Fluency in Spanish and/or Portuguese preferred.
Experience in diverse culture preferred.
Bilingual/bicultural preferred.
Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
Must have experience in diverse cultures, with strong commitment to promoting Diversity, Equity, and Inclusion and reducing inequities.
Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.
