Family Nurse Practitioner - Home Health Assessments

A large well known managed care organization has part-time (PT)  CONTRACT direct patient care family nurse practitioner opportunities in Montgomery OR,Bucks, OR  Chester in PA.

Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies

Must be enrolled in Medicaid in Pennsylvania to be submitted for consideration

These are both telehealth and in-person HRA  visits.

 Must be credentialed by Medicare, Medicaid, and other Private Insurance Companies.

HOURS:  8 hours a  day for 2 /3 days a  week 

CONTRACT DATES: now till the end of 2023 

PAY :

• $100.00 per completed assessment  in person 
• full-day training $400
• half-day training $200

PA  LOCATIONS: Montgomery County ,Bucks County ,Chester County 

QUALIFICATIONS : 

• Family Practice or  Internal Medicine  Nurse  Practitioners with  HRA/Hospice or  Home Health geriatric exp.
• CAQH
• Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies

• MUST already be enrolled in Medicaid in Pennsylvania to be submitted for consideration

• Board-certified by the ANCC or AANP

• Active/unrestricted license

• Must have current BLS certification

• Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies
  

• electronic medical records / electronic health records 

• proficiency with Microsoft Office (Logging into programs, Microsoft Word, Microsoft Excel, Microsoft Outlook, 

• Must have a valid state driver’s license, be able to drive your car, provide your own vehicle, have proof of adequate automobile insurance
  

• Highly motivated, self-directed professional with strong organizational skills and comfortable working independently on a daily basis

DUTIES :  

• Perform health risk assessments which are essentially history and physicals done within the patient’s home or on occasion in a nursing home or another health facility

• Requires clinical documentation of acute, chronic, and long-term healthcare issues through the patient encounters in EMR system

• Documentation must be accurate, high quality, and completed in a timely manner

• The provider will ensure all appropriate consent forms are signed and dated

• Provider will have frequent interaction with patients, coders, schedulers, and supervising physician

• The provider will educate patients and/or patient’s family on chronic medical conditions, preventative care, and medication adherence compliance

• Provider will complete referrals to case management, nursing, pharmacy, etc. as needed

• Provider will participate in clinical meetings and compliance trainings which are primarily web based; however, provider will occasionally be required to attend on-site training for which the provider will be reimbursed
