The Department of Family Medicine, Brody School of Medicine, East Carolina University seeks an innovative full-time faculty member for its developing Integrated Care Program. This is a transformative department, implementing integrated, team-based care in all aspects of education, teaching, and research.

This position will function as a member of the research team in the Center for Integrated Care Delivery in the implementation and conduct of several grant-funded projects, including:

  1. Prepare for Practice: Integration of Future Psychologists into Rural, Primary Healthcare Settings;
  2. Innovative training to transform rural care; and
  3. Healthier Lives at School and Beyond: Linking School-Based Health Centers to Expanded Resources for Care and Counseling.

As a part of these projects, This position will train and supervise graduate-level behavioral health trainees who are providing care to patients in an integrated primary care setting; contribute to the revision and restructure of a more unified, interdisciplinary curriculum across multiple professions in the primary care setting; and provide behavioral health services as a part of team-based care for underserved patients with diabetes and co-morbid disease/ behavioral challenges via a telehealth system directly into rural primary care practices at the time of the primary care visit.

This position will join other members of the Project Team within the Family Medicine Center.

This position offers the appropriate candidate the opportunity for clinical program research, model development, dedicated Family Medicine resident and clinical health psychology doctoral student teaching, and provision of clinical services within an integrated care/telehealth setting. Minimum qualifications include a completion of appropriate Doctoral Program (PhD) leading to current North Carolina license or eligibility as a Psychologist and Health Service Provider Psychologist, or Provisionally Licensed Psychologist in the state of North Carolina.

Qualifying degrees must be received from appropriately accredited institutions.

VIEW JOBS 11/29/2018 12:00:00 AM 2019-02-27T00:00 Collaborate with UNC Infectious Diseases Outpatient Parenteral Antimicrobial Therapy (OPAT) team, including, PharmD, Medical Director, MDs and Administration Assistance to ensure safe and coordinated care of patients receiving intravenous antibiotics post hospitalization. In addition, work closely with the Infectious Diseases inpatient and outpatient teams, the antibiotic stewardship program, discharge coordinators, vascular access team, home infusion pharmacies and visiting nurses. * Analyze and document weekly therapeutic drug levels, safety labs and inflammatory markers to ensure timely appropriate follow-up needed by PharmD and/or medical provider. * Perform telephone triage to determine need for urgent/emergent medical evaluation or intervention for issues such as, malfunctioning CVADs, acute signs and symptoms for worsening of infection, adverse drug reaction to IV antimicrobials, acute kidney failure * Receive incoming calls and faxes from dedicated OPAT phone and fax to address patient care issues in a timely manner * Provide Central Venous Access Devices (CVAD) patient education * Assess and problem solve malfunctioning CVADs * Perform CVAD dressing changes, instill Cathflow, blood withdrawals for labs ordered * Remove Peripherally Inserted Central Catheters (PICC) in clinic and provide patient education for site care at home * Infuse first dose IV antibiotics in clinic * Coordinate removal of tunneled central lines for patients on anticoagulants * Coordinate care with ID providers, home health agencies and Infusion companies to ensure smooth transitions in levels of care and changes in treatment plans with goals of safe, appropriate timely care that prevents avoidable re-hospitalization and ED visits. * Create OPAT nursing protocols and lead implementation efforts. * Present patient panel updates at weekly interdisciplinary team meetings. * Participate in design and leadership of OPAT-specific quality improvement initiatives, disseminate results in team meetings and other settings * Participate in the development and revision of interdisciplinary and nursing standards for OPAT team as well as patient education materials as relevant to area of practice and expertise. * Provide cross-coverage for ID clinic nursing team for HIV or other clinic patient nursing needs in event of an absence * Provide leadership for specific interventions for HIV patient population utilizing clinical case management skills. * Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. * Utilizes protocols, pathways and order sets to formulate, communicate, and ensure implementation of the patient plan of care. * Demonstrates flexibility to meet patient care needs. * Utilizes multidisciplinary colleagues to address individualized patient needs. * Medical records are completed promptly and accurately and reflect presence and usage of appropriate clinical pathway. * Acts as a resource to all levels of faculty, staff, and trainees by incorporating expertise and related experience into care of specific patient population. Preferred Qualifications: 1.Experience with management of home infusion patients and medically complex post-discharge patients 2.EMR documentation of care plans, phone calls, and in-person visits with patients 3.Quality improvement experience (design and implementation) 4.Strong writing and organizational skills 5.Experience working with diverse patient populations Requires graduation from a state-accredited school of professional nursing.
