APP Job Description

The Advanced Practice Practitioner is responsible for providing high quality primary medical care at the Duke Community Medicine Micro clinics as well as providing care to adolescent patients in the community through our School Based Health Center in Southern High School to extend the access of care amongst the community. Provider works under the supervision/in collaboration with the supervising physician within the scope of authority granted by the NC Board of Medical Directors.

Clinics serve a patient population that is historically underserved, un- or underinsured, and frequently non-English speaking, and works collaboratively with Lincoln Community Health Center (local FQHC). Spanish language proficiency preferred but not required.

Job Duties and Responsibilities

  • Provide high quality primary medical care for all patients, ages 2 and up, providing comprehensive medical care including preventative care, chronic disease management and acute care. The provider is responsible and accountable for independent patient load/clinic.

  • Coordinate patient care including triage, obtain health histories, complete physical examinations, collecting and documenting data, ordering laboratory and imaging studies, and prescribing appropriate interventions and medications.

  • Utilize prescriptive privileges as allowed by NC licensure to prescribe necessary treatment (both pharmacologic and non-pharmacologic) to include making appropriate medical referrals when necessary.

  • Communicate verbally and in writing all pertinent patient information to supervising physician, and nursing staff.

  • Coordinate patient care by communicating with medical providers from both within and outside our institution.

  • Provide timely documentation in the electronic medical record of all relevant data including results of diagnostic tests; laboratory results; patients' condition and response to therapies/interventions; communications with physician colleagues.

  • Respond to patient inquiries and concerns via telephone or EPIC MyChart messages and be accessible to consult with supervising physician as needed.

  • Analyze clinical practice patterns so as to provide the best medical care and to increase effectiveness and efficiency.

  • Role model professionalism through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, critical thinking, and problem-solving skills.

  • Maintain all documentation and certification required by law to practice medicine as a Physician Assistant or Nurse Practitioner in the state of North Carolina.

  • Participate in clinic and department meetings, safety protocols, and drills.

Physician Assistants: Must be licensed in the state of NC, and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants. BLS Required.

Nurse Practitioners: Must have current or unrestricted RN licensure issued by or recognized in the state of North Carolina Board of Nursing. BLS required.

Minimum Qualifications

Education

Work requires a bachelor's degree in Health Science and a Physician Assistant Certificate of Registration with the North Carolina Medical Board or be certification eligible, and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants.

Experience
