Diakon Family Life Services offers a range of counseling and behavioral health services to children, teens, families and adult individuals. Services are provided through counseling centers in Mechanicsburg, New Bloomfield, Lewisburg, Montoursville and Williamsport as well as within several public-school buildings.

Specialized programs include family-based mental health services, provided in the community and in-home, addictions counseling, trauma-focused therapy and SPIN, a service designed to assist children and youths who have problematic sexual behaviors. General Responsibilities:
Responsible for providing clinical services to individuals, couples, families, and groups in a co-occurring setting.# Develops individualized treatment plan for each client per agency procedures.# Provides a wide range of therapeutic interventions to individuals, couples, families and groups based on individualized assessment and treatment plans in conjunction with the treatment team.

Completes comprehensive psychological assessments on each client seen in keeping with professional standards of practice as well as DPW, DOH and other third party requirements. # This is a 9 month school-based part time position. Childline and DHS FBI results are required prior to beginning employment. Covid#Vaccine Mandate required.# Exemptions for medical and religious can be filed. # Education/Training: A Masters degree in psychology social work or counseling required; LMFT, LPC or LCSW preferred.

Experience:


A minimum of three (3) years post Masters supervised clinical experience in an outpatient setting normally required; experience in a drug and alcohol or co-occurring setting required.

The in-home setting could be a patient's private residence or in a patient's room inan independent living facility. Vitality to You allows the Physical Therapist to evaluate and treat in the most functional environment which is the patient's home. The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. He/she implements individualized therapy program designed to restore, reinforce, and enhance performance. In addition, the Physical Therapist instructs, educates and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. He/she directs activities to promote and maintain health. T his position provides a flexible schedule and work environment in various settings. PTHV 1. He/she must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility; and 2. He/she must have a Master's degree in Physical Therapy; or 3. He/she must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or 4. He/she must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience. 5. The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy. 6. He/she must have, or obtain within 90 days of employment, CPR certification per GRS standards. 7. He/she must have a valid driver's license. 8.
