Position Overview:

Public Health Solutions (PHS) is seeking 4 bilingual Family Liaison AmeriCorps members to support and enhance client navigation for PHS' Neighborhood Health Services. The Family Liaison AmeriCorps members will screen and connect families to the services they need, and provide programmatic and administrative support for program leadership and staff. The Member's role will include providing in-person and phone-based guidance to New Yorkers to connect to the programs, benefits, and resources that best fit their needs and interests, and helping to staff in-person resource distributions including the Queens Diaper Bank, and Queens Cribs for Kids programs. Full-time and part-time roles are available.

Specifically, the Family Liaison AmeriCorps Member will:

  • Screen participants and assist them with navigation to PHS and external services and resources.

  • Document all work done using trackers provided by PHS.

  • Provide information to participants in a manner that is culturally and linguistically appropriate to the needs of the population.

  • Manage referrals and screen clients through various referral data platforms including NowPow, Unite NYC, etc.

  • Coordinate and/or attend community events, workshops, and stakeholder meetings.

  • Conduct data entry and carry out routine office activities such as answering and routing phone calls, copying, filing, data entry, recordkeeping, handling incoming and outgoing e-mail, and mail, and other tasks as needed.

  • Provide in-person staffing for the Queens Diaper Bank, and the Cribs for Kids program.

Other responsibilities may include, depending on placement:

  • Provide information about PHS' services, basic program eligibility and required documents, appointments, locations, and wait times.

  • Screen and connect pregnant and parenting community members via phone call and closed-loop referral technology to PHS and other community partners' services, including home visiting, resources that impact the social determinants of health, and other parenting services. The Member will receive training and support to be successful in this role.

In addition, the Family Liaison AmeriCorps Member will:

  • Attend all required trainings and maintain current knowledge of information relevant to the program.

  • Participate in team meetings.

  • Handle other duties as assigned.

Qualifications/Requirements:

  • HS diploma or GED and some college coursework (enrolled in either an Associates or Bachelor's degree program).

  • Must be bilingual in English and another language such as Spanish, Mandarin/Cantonese, Haitian Creole, or Bengali.

  • Demonstrated strong interpersonal skills, cultural competency, and respectful interactions with community members looking for services. Must interact well with staff and clients alike and have an outgoing personality.

  • Ability to communicate clearly with patients, especially when communicating complex information

  • Demonstrated experience performing administrative duties with speed and accuracy without immediate and constant supervision.

  • Ability to maintain and improve upon tracking systems to promote smooth administrative operations.

  • Ability to learn quickly about PHS services, eligibility criteria, processes for applications, etc.

  • Ability to multi-task and prioritize needs of multiple programs.

  • Must be highly organized and detail oriented.

  • Ability to communicate effectively both orally and in writing with an attention to detail and professionalism in all communications.

  • Excellent team player with the ability to be flexible and work collaboratively and respectfully.

  • Previous experience with working with community partners and completing word and basic numerical reports.

  • Proficient in Microsoft Suite Applications (Word, Excel, SharePoint, etc.).

  • Ability to learn and navigate new data systems.

  • Demonstrated commitment to, and interest in learning more about, maternal child health, systems approach to health equity, and community services in NYC. Dedicated to helping improve the lives of disenfranchised and marginalized communities.

  • Salary: $2112.22- $35,200 bi-weekly allowance based on minimum hours worked.
