Full time Family Group Conferencing Coordinator opening in the Philadelphia area. Routine non-traditional hours. 40+ hours a week. Associates or Bachelor degree in social services or related field preferred and at least 3 years experience in the Human Services field. 8:00 am start time working with families in the Philadelphia area to empower them to make decisions and the best plan. Must have dependable transportation, valid PA drivers license, auto insurance and be able to obtain all the needed clearances( Child Line, FBI, PA State Police) If you are interested you MUST send cover letter with salary requirements, otherwise you will not be considered . VIEW JOBS 10/16/2018 12:00:00 AM 2019-01-14T00:00 Responsibilities: * Coordinate all activities between Interim House West and child welfare agencies, including the Department of Human Services. * Complete an initial intake on mom and children at time of admission to program. * Coordinate family reunification plans with other members of the treatment team as well as child welfare agencies. * Assist in compliance with licensing standards as set forth by federal, state and local authorities. * Convene weekly meetings between case managers, childcare director and nursing staff. * Coordinate family case management activities. * Develop and submit to DHS initial and follow-up service plans for all children funded through DHS. * Participate in Family Service Plan meetings or court appointments as necessary. * Complete quarterly reviews as required by local regulatory agencies. * Develop and submit to DHS quarterly progress reports. * Complete and submit discharge summaries for each child funded through DHS. * Coordinate activities with the Child Care Director. * Coordinate referrals to community-based children's educational, early intervention and mental health treatment services as necessary. * Collaborate with schools and other educational institutions in monitoring progress of children. * In collaboration with therapists, teachers, case managers and mothers, develop individualized service plans for identified children. * Coordinate referrals for children who are in need of individual and/or family therapy sessions. * Assist treatment team members with information for continuing care plans 1 month prior to discharge. * Record individual and group progress notes in the DDAP format after each activity. * Coordinate and supervise the activities of the teacher, teacher's assistants and teacher's aides. Education/Qualifications: * Completion of a graduate program at an accredited college or university with a Bachelor's degree in social work, early childhood education or human services field. * Experience with children experiencing delays in cognitive communicative and perceptual-motor skills development. * At least 2 years experience in providing case management services to children and/or families. * Experience working with the Department of Human Services, child welfare and related agencies. * Knowledge of community-based resources for children and families, who require specialized support.
