Family Advocate will work with families to promote family wellness & parent involvement programs to include advocacy, parent peer support group, and parent education and to support the implementation of partnership process. The Family advocate will operate as a liaison between the Clubhouse staff, youth and parents in the recovery support process.

Coordinating Services and Collaboration
Transport member to and from facility and/or community sites when needed
Meet with family members monthly
Monitor, schedule, facilitate and/or participate in Team Meetings for members.
Collaborate and communicate with other programs in the CHRIS 180 continuum in the provision of services to members
Collaborate and consult with other providers and entities in the provision of best care services to members
Monitor the provision of services provided.
Assessing client and family needs based on history of trauma and strengths.
Coordinating and collaborating services with team (form group activities, transporting clients, home visit appointments, monthly parent education group, family outings etc.)
Management of caseload and upkeep of documentation in Care-Logic.
Satisfy all state, agency and contract guideline.
General check ins with and their families weekly.
Locate information and other resources to assist clients and families with urgent needs
Facilitate parent education group and provide resources
Pick up and drop off youth for community service and group outings
Collaborate with team to maintain upkeep of caseload
Find online trainings and essential learning to assist with services and trauma-informed care
Satisfy all state and agency guidelines.
Provide member updates in Case Notes within required timeframe.
Complete Incident Reports as needed prior to shift end
Monitor completion of deadlines (i.e Monthly Summary Reports)
Provide reports to governing/referral agencies regarding participation of member
Performance and Conduct
Maintain confidentiality of member information
Maintain appropriate professional boundaries
Follow the CHRIS 180 Code of Ethics
Follow mandated reporting requirements of GA Code
Support and maintain an environment of emotional and physical safety for members, staff and other stakeholders always

Maintain Personal Information and Training;
Attend and participate in staff meetings as scheduled

Provide documents in a timely fashion to Human Resources
Report changes in status to HR as required by policy
