Description The Facilitator coordinates the development of Child and Family Teams (CFTs), maximizing the involvement of natural team members and system partners.
The facilitator guides the CFT through the plan development process using accepted High-Fidelity Wraparound technologies and techniques.
The Facilitator prepares individualized family plans and coordinates each plans implementation, encouraging the maximum use of natural resources and community-based services; supplementing these with flexible Wraparound staffing and ethical use of available funding.
The Facilitator functions as the case manager, representing the CFTs progress through the Wraparound process in written documentation and verbal presentations.
The facilitator supports the Mission, Vision, and Values of Stanford Youth Solutions.
Qualifications Masters degree in Social Work, Counseling, Psychology with LMFT/AMFT or LCSW/ASW license or license eligibility OR Masters degree in related field and at least 2 years of approved experience* OR Bachelors degree in a related field and at least 4 years approved experience* (*approved experience is experience that qualifies the applicant for Mental Health Rehabilitation Specialist (MHRS) status in the Sacramento County Childrens Mental Health System of Care) Prepared to work independently with flexible hours that may include evenings, weekends and holidays Proven documentation skills and intermediate level computer skills including all MS Office programs Adhere to all confidentiality requirements Verbal and written ability to communicate effectively with colleagues, community partners, consumers and all levels of management Ability to work in a mobile, fast paced environment.
Ability to use a laptop computer Ability to multi-task and prioritize projects and assignments effectively Salary Hiring Range : $18.03-$22.63 hourly

The Lean Consultant will be part of a team responsible for teaching and coaching administrators, clinicians and front-line staff on the application of Lean to solve organizational problems. The Lean Consultant leads and facilitates improvement projects with some coaching, including Kaizen Events, Value Stream Mapping sessions, and other kaizen events, and contributes to the development and implementation of improvement programs across the organization. Consults to and coaches process owners, value stream owners, paired leaders and sponsors , as well as front line staff. Contributes to the development and execution of training and infrastructure for the Lean Promotion Office. Principal Accountabilitites Consults to and coaches process owners, value stream owners, paired leaders and sponsors in event and workshop activities, as well as front line staff on Lean process improvement tools and methods and on the development of a Lean Management system. Leads and facilitates improvement events and projects, including assessment, planning, event facilitation, and follow-up/sustaining, with some coaching. Meets quality standards and the production of expected deliverables during all phases of the event and project life cycle. In concert with project sponsors and senior consultants, assesses & identifies problems and opportunities, coordinates the activities of the improvement team, identifies appropriate resources needed, develops schedule to ensure timely completion of improvement project. Supports the development of Lean Promotion Office infrastructure, including Lean "Best Practices", tools & methods for Lean education and promotion of Lean thinking throughout the organization. Educates employees and leadership on Lean principles using various outlets such as New Employee or Manager orientations, event-based training, and other formal and informal educational opportunities. Champions the Lean culture throughout all interactions. Coaches process owners to track and report results from improvement efforts. May collect and analyze data, maintain results and assist in preparing reports to support decision-making and continuous improvement. Is able to translate data into meangful information, make conclusions, and shape work at the department level. Provides leadership for improvement programs for departments and project team members by defining intent of programs and overseeing development deliverables to ensure project goals are met. In concert with project sponsors and senior consultants, assesses & identifies problems and opportunities, assembles improvement team, identifies appropriate resources needed, develops schedule to ensure timely completion of improvement project. Establishes effective working relationships & manages communications with project sponsors, project oversight group members, process owners & key stakeholders. Serves as a mentor and coarch to other LPO staff to grow their Lean technical and consultating skills. May also provide training during the on-boarding stages for newly hired colleagues. Performs other duties and responsibilities as assigned. Education/Experience: BA/BS or Equivalent education/experience required. Lean certification desired. Requires a general knowledge of business operations and process improvement activities as typically acquired with 1-3 years relevant work experience. Experience with Lean process improvement tools and methods or Lean certification desired. Healthcare experience strongly preferred. Highly respected team player/learner. Possesses a strong desire and ability to rapidly learn and apply the principles of a Lean enterprise. Able to perform complex tasks, prioritize multiple projects, and work under pressue to meet deadlines in a fast-paced environment. Able to read & interpret data; possesses strong analytical and critical thinking skills. Strong verbal and written communication skills. Must be comfortable speaking in public and to various audiences. Proficient in all applications of Microsoft Office, with an emphasis on Excel, Powerpoint, Visio, Word and Outlook. Knowledge of and experience using Lean tools and concepts preferred, such as: PDSA and other structured problem solving tools, A3 thinking, Daily Problem Solving at the front line, Waste, 5S and Standard Work. Understanding of healthcare industry & terminology preferred. Skills in project management, teaching and training, consulting with local leadership, group facilitation desired.
