Facilitator Service VA

Position Summary: Provides supportive services designed to prevent or reduce inappropriate institutional care by offering assistance to eligible recipients and the hiring, training and supervising responsibilities of the personal care aides, who perform basic health-related services to EDCD Waiver recipients.

Essential Duties:

• Ensure that clients are receiving the services needed and that they are on a current service plan.

• Review required documents with the client, family and caregiver, as appropriate.

• Train the caregivers providing care for consumer directed clients on the required tasks of an employer.

• Develop service plans and complete appropriate Company/State required paperwork.<

• Provide service facilitation services in accordance with DMAS regulations, policies, and guidelines.

• Oversee service plans to ensure the client's health, safety and personal autonomy.

• Maintain proper documentation and records to show client's continued need for participation in the Virginia CS Services Program(s).

• Performs other duties as assigned

Qualifications:

• Possess a working knowledge of activities of daily living (ADL's) and instrumental activities of daily activities (IADL's).

• Must have one year of satisfactory experience in the human services field working with people with severe disabilities or the elderly.

• Have the ability to work independently

• Must have a vehicle with current insurance coverage and a valid state driver's license

• Must have good organization skills, as well as oral and written communication skills
