Overview

Are you eligible to, or certify as a Mental Health Rehabilitation Specialist (MHRS)? If you are, become our new Facilitator for our Sonoma County Child Abuse Prevention Services (CAPS) Wraparound services and join us in providing family-centered and strength-based services!
As a Facilitator for Wraparound Services, you will provide culturally responsive services and leads a collaborative planning process for youth and families who are referred through Child Welfare, Juvenile Probation, or County Behavioral Health departments. Services will be provided almost exclusively in the community: in families' homes, foster homes, schools, other community locations, etc. for youth who are at-risk of being placed in out-of-home care or are stepping down from a higher level of care. Your primary role is to ensure a sustainable network of unpaid supports (i.e. extended family, mentors, teachers, coaches) are working together toward stability for the youth and family.
ABOUT THE PROGRAM
The Sonoma County Child Abuse Prevention Services (CAPS) Wraparound Program is an intensive home and community-based service offered to youth and families identified by Child Welfare as high risk for entering the dependency system. Service objectives include safety planning and stabilization of the home environment via community resourcing, short term youth and parent skills coaching, engagement + activation of natural supports, and referrals to other services/therapeutic treatment as needed to increase safety and divert youth from ever entering the foster care system. Services duration is 1-3 months for approximately 7-9 youth on average.

Responsibilities
Facilitation of Child and Family Team Meetings. This includes preparing for the meeting, coordinating attendance, facilitating the meeting, writing meeting notes and distributing them within two days, and writing mental health notes
Provide clinical case management and coordination of care
Collaborate with county and community agency partners, including but not limited to county social workers, probation officers, outside therapists, psychiatrists, teachers, etc
Intervene with clients' behavior management programs and provide crisis management as needed
In circumstances where there is serious threat of harm, this may include physical management/restraint of the clients (implementation of Seneca's Equilibrium model)
Complete comprehensive assessments, treatment plan of care, mental health notes, Family Team Meeting notes, Plan of Care, incident reports, and other documentation as necessary in a thorough, accurate and timely manner
Support clients' behavior management programs, provide collateral and permanency services, and crisis management as needed.
Qualifications
REQUIRED:
Bachelor's degree, in Counseling/Psychology/Social Work
4 years experience working with children/youth and families with complex and enduring needs in a mental health setting
Or Master's degree in Human Services, Social Services or related field and two years of experience in Mental Health settings
MHRS certification is required, in accordance with county contracts for services
Willing to respond to crisis situations and be part of a rotating emergency pager system, scheduled ahead of time
Must be at least 21 years of age
Own vehicle, Valid California driver's license and insurability with Seneca's automobile policy
TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
Must show proof of COVID-19 vaccination
PREFERRED
Bilingual in Spanish skill set preferred
Experience working with youth in out-of-home care preferred
Schedule
Full-time
Mon - Fri; 9AM - 5:30PM
Requires flexible schedule that includes early mornings, evenings, and occasional weekends as needed
Participate in providing emergency on-call response as scheduled
Benefits
Starting at $31.18 - $33.68 per hour
As of July 7th, this starting range will increase to $33.82 - $36.32
Actual salary is dependent on creditable experience above the minimum qualifications for the role
Additional compensation provided upon passing bilingual language proficiency exam
Salary increases each year
Stipends provided to staff members participating in the emergency on-call shifts
